摘  要:目的 探讨白细胞介素-9(IL-9)基因多态性和垂体瘤术后继发颅内感染的危险因素。方法 回顾性选取2020年1月-2023年1月泉州市第一医院收治的垂体瘤术后继发颅内感染患者34例作为病例组,另选取同期垂体瘤术后未继发颅内感染患者80例作为对照组,统计两组IL-9基因多态性、临床资料以及病原菌感染特点,采用单因素及多因素分析发生术后颅内感染的易感因素。结果 病例组IL-9 rs1859430位点AA基因、A等位基因频率高于对照组,G等位基因低于对照组(P<0.05);34例垂体瘤术后继发颅内感染患者中,脑膜炎27例,脑室炎7例;共分离出病原菌45株,革兰阳性菌28株(62.22%),革兰阴性菌16株(35.56%),真菌1株(2.22%);采用多因素Logistic分析,肿瘤直径>3 cm(OR=3.664,P=0.022)、术后脑脊液漏(OR=4.267,P=0.011)、IL-9 rs1859430位点AA基因(OR=4.505,P=0.005)均为影响垂体瘤术后继发颅内感染的独立危险因素。结论 IL-9 rs1859430位点A基因是垂体瘤术后继发颅内感染的易感基因,临床应重点观察肿瘤直径>3 cm、术后脑脊液漏等因素,减少术后颅内感染的发生率,改善患者预后。OBJECTIVE To explore the IL-9 gene polymorphism and risk factors for intracranial infection after pituitary tumor surgery.METHODS A total of 34 patients with intracranial infection after pituitary tumor surgery in Quanzhou First Hospital between Jan 2020 and Jan 2023 were retrospectively enrolled as the case group,while other 80 patients without intracranial infection during the same period were enrolled as the control group.The IL-9 gene polymorphism,clinical data and characteristics of pathogens infection between both groups were statistically analyzed.The susceptibility factors for postoperative intracranial infection were analyzed by univariate and multivariate analysis.RESULTS The frequency of AA gene and A allele at IL-9 rs1859430 locus was higher and G allele was lower in the case group than those in the control group(P<0.05).Among the 34 patients with secondary intracranial infection after pituitary tumor surgery,meningitis occurred in 27 cases and ventriculitis occurred in 7 cases.There were 45 strains of pathogens detected,including 28 strains of gram-positive bacteria(62.22%),16 strains of gram-negative bacteria(35.56%)and 1 strain of fungus(2.22%).Binary logistic analysis showed that Tumor diameter>3 cm(OR=3.664,P=0.022),postoperative cerebrospinal fluid leakage(OR=4.267,P=0.011),IL-9 rs1859430 AA gene(OR=4.505,P=0.005)were independent risk factors for secondary intracranial infection after pituitary tumor surgery.CONCLUSION A gene at IL-9 rs1859430 locus is a susceptibility gene for secondary intracranial infection after pituitary tumor surgery.Clinical observation should focus on risk factors such as tumor diameter>3 cm,and postoperative cerebrospinal fluid leakage to reduce the incidence of postoperative intracranial infection to improve prognosis.
