中医药治疗艾滋病发热之临床体会

摘　　要：古今中医各家认为,艾滋病发热的病因病机主要由艾毒、风、热、湿等六淫外邪,或情志因素作为诱因,内外相引,由里越外,或外感,或内伤,最终导致肺脾肾等五脏六腑功能失调,气血阴阳俱虚,总属本虚标实,正邪相争。经临床实践,认为艾滋病发热的病因病机临床主要表现为内外湿交杂,湿热蕴结,困遏脾土,脾失健运,加之艾滋病患者脾为枢机,脾气本虚,最终导致湿热为标、脾虚为本的病理状态,病机总属本虚标实,正邪相争,虚实夹杂。临床常见证候有外寒里热证、湿热内蕴证、热毒炽盛之热在肺卫证、气虚发热证,其中湿热内蕴证和(脾)气虚发热证最为多见。通过对艾滋病发热病因病机理论和中医证候的总结,从而剖引中医药治疗艾滋病发热的辨治思路和临证体会,以期对增强临床疗效提供帮助。Ancient and modern traditional Chinese medicine practitioners believe that the etiology and pathogenesis of acquired immune deficiency syndrome(AIDS)fever is mainly caused by six exogenous evils,such as AIDS virus,wind,heat and dampness,or emotional factors,or external factors,or internal injuries,ultimately leading to the dysfunction of the five Zamg-Fu organs,such as lung,spleen and kidney,and the deficiency of Qi,blood,Yin and Yang.It belongs to the type of deficiency in origin and excess in superficislity and the competition between the vital Qi and evils.Through clinical practice,we believe that the main etiology and pathogenesis of AIDS fever are internal and external dampness,accumulation of dampness and heat damaging spleen,loss of transportation of spleen.In addition,the spleen of AIDS patients is the cardinal and the spleen is inherently weak,which ultimately leads to dampness and heat and spleen deficiency.The pathogenesis is always the deficiency in origin and excess in superficislity,the competition between the vital Qi and evils,and the deficiency and the actual are mixed.Common clinical syndromes include external cold and internal heat syndrome,damp-heat internal accumulation syndrome,and flaming heat in lung health syndrome,Qi deficiency and fever syndrome,of which damp-heat internal accumulation syndrome and(spleen)Qi deficiency and fever syndrome are the most common.The article summarized the etiology and pathogenesis theory of AIDS fever and TCM syndromes,so as to introduce the differentiation and treatment ideas and clinical experience of TCM treatment of AIDS fever,in order to provide help for enhancing the clinical efficacy.
