清筋术联合紫朱软膏外用治疗糖尿病足筋疽的临床疗效

摘　　要：目的探讨清筋术联合紫朱软膏外用治疗糖尿病足筋疽的临床疗效。方法将符合纳入标准的60例糖尿病足筋疽患者,随机分为观察组30例和对照组30例,两组给予基础治疗的同时,观察组给予清筋术联合紫朱软膏外敷,对照组给予清创术联合纳米银医用抗菌敷料外敷,比较两组临床疗效、创面肉芽形态、创面疼痛症状及炎症因子变化。结果观察组临床疗效明显优于对照组(P<0.05),治疗后观察组肉芽组织形态评分、疼痛程度VAS评分明显低于对照组(P<0.05),观察组C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、降钙素原(PCT)均较治疗前显著降低(P<0.05),其中CRP、IL-6、TNF-α明显低于对照组(P<0.05)。结论清筋术联合紫朱软膏外用治疗糖尿病足筋疽临床疗效确切。Objective To explore the clinical efficacy of therapy of clearing sinew combined with Zizhu Ointment(紫朱软膏)for treating diabetic foot sinew gangrene.Methods Sixty diabetic foot sinew gangrene cases who met the inclusion criteria were randomly divided into observation group(30 cases)and control group(30 cases).Based on the basic treatment,the observation group was administered the therapy of clearing sinew combined with Zizhu Ointment for external application,and the control group was administered conventional surgical technique combined with nano-silver medical antibacterial dressing.The clinical efficacy,wound granulation morphology,wound pain symptoms and changes in inflammatory factors between two groups were observed.Results The clinical effect of the observation group was better than that of the control group(P<0.05).After treatment,the granulation tissue morphology score and visual analog scale(VAS)score in the observation group were significantly lower than those in the control group(P<0.05).The levels of C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and Calcitonin(PCT)in the observation group were significantly lower than those before treatment(P<0.05),and the levels of CRP,IL-6 and TNF-α were significantly lower than those in the control group(P<0.05).Conclusion The therapy of clearing sinew combined with Zizhu Ointment has a good curative effect on diabetic foot sinew gangrene.
