中药配合病灶切除缝合术治疗肉芽肿小叶性乳腺炎的临床研究

摘　　要：目的观察中药配合病灶切除缝合手术,治疗肉芽肿性小叶性乳腺炎的临床效果。方法通过回顾性的队列研究,将经病理诊断为肉芽肿性小叶性乳腺炎的157例病人,经过倾向评分匹配的80例患者纳入组(中药配合病灶切除缝合术组为40例,单纯中药为40例),比较两组6个月、9个月治愈率、治愈时间、治疗病程等情况。结果中药配合病灶切除缝合术组和单纯中药组组间治疗期间化脓情况比较(60.0%vs 82.5%,P<0.05),治疗6个月及9个月时中药配合病灶切除缝合术组治愈率明显优于单纯中药组(P<0.05);中药配合病灶切除缝合术组治愈时间及治疗总病程均短于单纯中药组(P<0.05)。结论综上所述,中药配合病灶切除缝合术治疗可以适当的提高治愈率,缩短治愈时间,适当的手术时机可以避免肉芽肿性小叶性乳腺炎肿块进一步酿脓,从而缩短病程,减少痛苦。Objective The clinical effect of traditional Chinese medicine combined with modern lestectomy and suture surgery to treat granulomatous lobular mastitis was investigated.Methods Through a retrospective cohort study,157 patients who were pathologically diagnosed as granulomatous lobular mastitis in the hospital,and 80 patients who were matched by propensity score matching were included in the Chinese medicine with lestectomy and suture surgery group(40 patients)and the Chinese medicine alone group(40 patients).The cure rate,cure time and course of treatment of the two groups were compared in 6 and 9 months.Results The comparison of suppuration during treatment between the Chinese medicine with lestectomy and suture surgery group and the Chinese medicine alone group had significance(60%vs 82.5%,P<0.05).The cure rate of the Chinese medicine with lestectomy and suture surgery group at 6 and 9 months of treatment was significantly better than that of the Chinese medicine alone group(P<0.05).The cure time and total duration of treatment in the Chinese medicine with lestectomy and suture surgery group were shorter than those of the Chinese medicine alone group(P<0.05).ConclusionIn summary,traditional Chinese medicine combined with lestectomy and suture surgery can appropriately increase the cure rate and shorten the cure time.Proper operation timing can prevent the granulomatous lobular mastitis from further pus formation so as to shorten the course of the disease and reduce the pain.
