极泉穴水针注射治疗臂丛神经痛的临床探索

摘　　要：目的观察极泉穴水针注射应用于臂丛神经痛(brachial plexus neuralgia,BPN)患者中的效果。方法前瞻选取2020年6月—2021年6月海安市中医院针灸科接收60例BPN患者为研究对象,根据门诊号单双号,将单号患者纳为对照组(30例),将双号患者纳为观察组(30)例;对照组患者接受针刺治疗;观察组患者接受极泉穴水针注射治疗;5 d为1个疗程,共治疗4个疗程,比较两组治疗效果;于治疗前、治疗4个疗程,比较两组睡眠质量[采用匹茨堡睡眠质量指数量表(PSQI)评价]及生活质量[采用世界卫生组织生活质量测定简表(WHOQOL-BREF)评估]。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05);治疗4个疗程,两组PSQI评分较治疗前下降,且观察组低于对照组,两组WHOQOL-BREF量表中躯体健康、心理状态、社会关系、环境、综合维度评分均较治疗前升高,且观察组均高于对照组,差异有统计学意义(P<0.05)。结论极泉穴水针注射应用于BPN的治疗效果确切,更利于减轻患者疼痛感,提高睡眠质量,进而改善患者生活质量。Objective To observe the effect of hydro-acupuncture injection at Jiquan(HT1)in the application of patients with brachial plexus neuralgia(BPN).Methods Sixty patients with BPN in Department of Acupuncture,Hai’an Hospital of Traditional Chinese Medicine from June 2020 to June 2021 were selected as the research subjects,according to the odd or even number of outpatient number.The patients with odd number were included in the control group(30 cases),and those with even number were included in the observation group(30 cases).The patients in the control group were treated with acupuncture.Those in the observation group were treated with hydro-acupuncture injection at Jiquan(HT1).5 days was a course of treatment,there were a total of 4 courses of treatment.Before treatment and after 4 courses of treatment,the sleep qualities[evaluated by Pittsburgh sleep quality index(PSQI)and assessed by the World Health Organization short form for quality of life(WHOQOL-BREF)]of the two groups before treatment and after 4 courses of treatment were compared.Results The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After 4 courses of treatment,the PSQI score of both groups were decreased,and the score of the observation group was lower than that of the control group.The scores of physical health,mental state,social relationship,environment and comprehensive dimension in WHOQOL-BREF were increased in both groups than those before treatment,and the scores of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion Hydro-acupuncture injection at Jiquan(HT1)has certain efficacy in the treatment of BPN,which is more conducive to reducing the pain of patients,improving the quality of sleep,and then improving the quality of life of patients.
