摘　　要：目的探讨孕通汤联合宫腔镜下输卵管插管通液术治疗输卵管性不孕患者的疗效。方法选取100例2019年1月—2021年1月该院收治的输卵管性不孕患者分为对照组和观察组,分别50例,采用随机数字表法的分组方式。两组均给予宫腔镜下输卵管插管通液术,术后给予常规抗感染和促排卵治疗,在直径超过18 mm的卵泡1~2个时应用人绒毛膜促性腺激素8000~10000 U促进卵泡破裂,同时指导患者同房,观察组另采用孕通汤连续治疗2个疗程,且两组均随访12个月。比较两组治疗后的临床疗效,治疗前后的血清卵泡刺激素(FSH)、雌二醇(E_(2))、促黄体生成素(LH)水平、输卵管通畅度及治疗后12个月的妊娠情况。结果治疗后,观察组临床总有效率(94.00%,47/50)与对照组(76.00%,38/50)相比较高;两组治疗后血清FSH、LH水平与治疗前相比均降低,与对照组进行比较,观察组均更低;两组治疗后通而不畅率、单侧阻塞率、双侧阻塞率降低;两组治疗后输卵管通畅率及血清E_(2)水平均升高,与对照组进行比较,观察组均更高;治疗后12个月内观察组宫内妊娠率高于对照组,差异有统计学意义(P<0.05)。结论孕通汤联合宫腔镜下输卵管插管通液术治疗输卵管性不孕患者可有效调节性激素水平,提高输卵管通畅率和宫内妊娠率,临床效果显著。Objective To explore the clinical efficacy of hysteroscopic tubal intubation combined with Yuntong Decoction(孕通汤)in the treatment of tubal infertility.Methods A total of 100 patients with tubal infertility who were treated in the hospital from January 2019 to January 2021 were selected and divided into the control group and the observation group by using the random number table method,50 cases in each group.Both groups were given hysteroscopic fallopian tube intubation,routine anti-infection and ovulation induction treatment after operation,and when there were 1-2 follicles with a diameter of more than 18 mm,8000-10000 units of human chorionic gonadotropin were used to promote follicle rupture and the observation group was treated with Yuntong Decoction for 2 consecutive courses.Both groups were followed up for 12 months.The clinical efficacy after treatment,the serum levels of follicle stimulating hormone(FSH),estradiol(E_(2))and luteinizing hormone(LH)before and after treatment,tubal patency and 12-month pregnancy after treatment were compared between the two groups.Results After treatment,the total clinical effective rate(94.00%,47/50)of the observation group was higher than that of the control group(76.00%,38/50).The levels of serum FSH and LH in the two groups after treatment were lower than those before treatment,and the levels of the observation group were lower.The rate of lateral occlusion and bilateral occlusion decreased.After treatment,the patency rate of fallopian tubes and serum E_(2) levels in the two groups were increased,and those of the observation group were higher.The intrauterine pregnancy rate of the observation group was higher than that of the control group within 12 months after treatment(P<0.05).Conclusion Yuntong Decoction combined with hysteroscopic tubal intubation can effectively regulate the levels of sex hormones and improve the rate of tubal patency and intrauterine pregnancy in patients with tubal infertility,with significant clinical effects.
