温阳行气法联合舍曲林治疗心肾阳虚气滞型抑郁症80例临床观察

摘　　要：目的对比分析温阳行气法联合舍曲林和单独使用舍曲林治疗心肾阳虚气滞型抑郁症患者的应用价值。方法选取心肾阳虚气滞型抑郁症病例80例,按随机数字表法分为两组,每组40例,比较分析两组的疗效、中医证候积分、PHQ-9健康问卷(PHQ-9)、汉密尔顿抑郁量表(HAMD)、血清白细胞介素-6(IL-6)、白细胞介素-10(IL-10)表达水平以及药物不良反应情况。结果观察组患者的总有效率显著高于对照组(P<0.05);治疗后观察组患者的情绪低落、睡眠障碍以及焦虑等中医证候积分显著低于对照组(P<0.05);治疗后观察组患者的HAMD和PHQ-9评分显著低于对照组(P<0.05);治疗后观察组患者血清的IL-6水平显著低于对照组(P<0.05),IL-10水平显著高于对照组(P<0.05);两组患者治疗期间总药物不良反应率无显著差异(P>0.05)。结论温阳行气法结合舍曲林治疗心肾阳虚型抑郁症患者的治疗效果要优于只使用舍曲林,可以有效减少患者的情绪低落、睡眠障碍以及焦虑等症状,缓解患者的炎症反应情况,并且不会增加药物不良反应。Objective To compare and analyze the application value of warming Yang and activating Qi movement method combined with sertraline and sertraline alone in the treatment of patients with depression of heart-kidney Yang deficiency and Qi stagnation.Methods A total of 80 patients with depression of heart-kidney Yang deficiency and Qi stagnation were enrolled and divided into two groups according to random number table method,40 cases in each group.The curative effect,scores of TCM syndromes,Patient Health Questionnaire-9(PHQ-9)and Hamilton Depression Scale(HAMD),levels of serum interleukin-6(IL-6)and interleukin-10(IL-10)and adverse reactions were compared between the two groups.Results The total response rate in the observation group was significantly higher than that in the control group(P<0.05).After treatment,the scores of TCM syndromes(depression,sleep disorders,anxiety)in the observation group were significantly lower than those in the control group(P<0.05),the scores of HAMD and PHQ-9 were significantly lower than those in the control group(P<0.05),the level of serum IL-6 was significantly lower than that in the control group(P<0.05),and the IL-10 level was significantly higher than that in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups during treatment(P>0.05).Conclusion The curative effect of warming Yang and activating Qi movement method combined with sertraline is better than that of sertraline alone on patients with depression of heart-kidney Yang deficiency and Qi stagnation,which can effectively reduce depression,sleep disorders and anxiety and relieve inflammation response,without increasing adverse drug reactions.
