养阴和血解毒方熏蒸坐浴用于肛瘘术后的研究

摘　　要：目的观察养阴和血解毒方熏蒸坐浴用于肛瘘术后的治疗价值。方法选取2018年1月—2020年12月于该院收治300例单纯性低位肛瘘为研究对象,采用随机信封抽签法分为观察组(151例)和对照组(149例)。对照组术后进行高锰酸钾熏蒸坐浴治疗,观察组在对照组基础上给予养阴和血解毒方熏蒸坐浴干预。比较两组肛瘘治疗总有效率、伤口渗出液评分、肉芽形态评分、伤口愈合率。结果两组患者治疗总有效率比较差异无统计学意义(P>0.05),观察组伤口渗出液术后3、5、7 d评分显著低于对照组,差异有统计学意义(P<0.05),观察组术后伤口3、5、7、14 d肉芽形态评分显著低于对照组,差异有统计学意义(P<0.05),观察组7、14、21 d伤口愈合率显著高于对照组,差异有统计学意义(P<0.05)。结论养阴和血解毒方熏蒸坐浴用于肛瘘术后治疗,能减少伤口渗出液,促进健康肉芽生成,加快伤口愈合。Objective To observe the therapeutic value of Yangyin Hexue Jiedu Decoction(养阴和血解毒方)for fumigation sitz bath after anal fistula surgery.Methods A total of 300 patients with simple low anal fistula treated in the hospital between January 2018 and December 2020 were selected as the research subjects,and they were divided into the observation group(151 cases)and the control group(149 cases)by the random envelope drawing method.The control group was given potassium permanganate fumigation sitz bath after surgery,while the observation group was treated with fumigation sitz bath intervention with Yangyin Hexue Jiedu Decoction on the basis of the control group.The total effective rate of anal fistula treatment,wound exudate score,granulation morphology score and wound healing rate were compared between the two groups.Results There was no significant difference in the total effective rate of treatment between the two groups(P>0.05).The wound exudate scores in the observation group at 3,5 and 7 days after surgery were significantly lower than those in the control group(P<0.05).The granulation morphology scores at 3,5,7 and 14 days after surgery were significantly lower in the observation group than those in the control group(P<0.05),and the wound healing rates at 7,14 and 21 days after surgery were significantly higher than those in the control group(P<0.05).Conclusion Fumigation sitz bath with Yangyin Hexue Jiedu Decoction for postoperative treatment of anal fistula can reduce the wound exudate,promote the healthy granulation and accelerate the wound healing.
