栀子厚朴汤治疗老年COPD合并睡眠障碍的研究

摘　　要：目的探讨栀子厚朴汤对老年慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)合并睡眠障碍的疗效。方法选取2016年1月—2021年12月在该院进行治疗的90例老年COPD合并睡眠障碍患者,按照随机数字表法分为对照组与研究组,各45例。对照组予以常规治疗,研究组在对照组基础上加以栀子厚朴汤,两组均治疗4周。比较两组临床疗效;评估两组治疗前后中医证候积分、匹兹堡睡眠质量指数(PSQI)、慢性阻塞性肺疾病评估测试得分(CAT)及肺功能[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、FEV1/FVC]。结果治疗前,两组中医证候积分、PSQI维度得分、FEV1、FVC、FEV1/FVC及CAT得分差异均无统计学意义(P>0.05);治疗后,研究组治疗总有效率、FEV1、FVC、FEV1/FVC高于对照组,中医证候积分、PSQI维度得分及CAT得分均低于对照组,差异具有统计学意义(P<0.05)。结论栀子厚朴汤治疗COPD合并睡眠障碍具有较好疗效,不仅明显改善睡眠质量,还可减轻COPD病情,提升患者肺功能。Objective To investigate the efficacy of Zhizi Houpo Decoction(栀子厚朴汤)in the treatment of elderly patients with chronic obstructive pulmonary disease(COPD)and sleep disorders.Methods A total of 90 elderly patients with COPD and sleep disorders who were admitted to the hospital from January 2016 to December 2021 were selected.They were divided into control group and study group by random number table method,with 45 cases in each group.All patients were given routine treatment.On this basis,the patients in the study group were treated with Zhizi Houpo Decoction.Both groups were given 4 weeks of treatment.The two groups were compared in terms of clinical efficacy,TCM syndrome scores,Pittsburgh Sleep Quality Index(PSQI)scores,COPD Assessment Test(CAT)scores and pulmonary function[forced expiratory volume in 1 second(FEV1),forced vital capacity(FVC)and FEV1/FVC].Results There was no statistically significant difference between the two groups in terms of TCM syndrome scores,PSQI scores,FEV1,FVC,FEV1/FVC or CAT scores before treatment(P>0.05).After treatment,the total treatment response rate,FEV1,FVC and FEV1/FVC in the study group were higher than those in the control group,while TCM syndrome scores,PSQI scores and CAT scores were lower than those in the control group(P<0.05).Conclusion Zhizi Houpo Decoction is effective in the treatment of COPD complicated with sleep disorders,and can not only significantly improve the patients'sleep quality but also alleviate the condition and improve the pulmonary function.
