摘 要:目的研究参附注射液(SFI)对急性心肌梗死(acute myocardial infarction,简称“心梗”AMI,)模型大鼠心肌形态学及凋亡的保护作用机制。方法按照随机数字表法将40只SD大鼠分为假手术组、模型组、缬沙坦组和SFI组,每组10只。采用左冠状动脉前降支结扎法建立AMI模型,造模成功后,给予相应药物每日1次,连续14 d。应用超声仪观察各组大鼠心室壁运动变化情况,并评估心功能。结合苏木精-伊红染色观察心肌形态学变化,采用天狼星红染色法考察梗死边缘区Ⅰ、Ⅲ型胶原比例改变情况。利用免疫组化染色法(IHC)联合逆转录聚合酶链式反应(RT-PCR)检测心肌凋亡因子Bcl-2相关X蛋白(Bax)、B淋巴细胞瘤-2基因(Bcl-2)及半胱氨酸天冬氨酸蛋白酶-3(Caspase-3)的表达水平。结果AMI组大鼠左心室射血分数(LVEF)、短轴缩短率(LVFS)较假手术组显著降低(P<0.01),左心室前壁变薄,运动减弱,心腔扩张程度明显。形态学结果显示,AMI大鼠心肌结构破坏严重,心肌残存细胞少,Ⅰ、Ⅲ型胶原比例显著增加(P<0.01)。心肌中Bax/Bcl-2及Caspase-3 mRNA表达水平均明显升高(P<0.01)。SFI可明显改善AMI大鼠心脏结构,增加左心室壁厚度和运动,提升LVEF、LVFS水平,降低梗死边缘区Ⅰ、Ⅲ型胶原比例并下调Bax/Bcl-2及Caspase-3 mRNA表达(P<0.05,P<0.01)。结论SFI可通过改善心功能,减轻心肌损伤,抑制胶原生成和抗凋亡等方面发挥其保护AMI后心脏作用。Objective To investigate the protective mechanism of Shenfu Injection(参附注射液,SFI)on myocardial morphology and apoptosis in rats with acute myocardial infarction(AMI).Methods Totally 40 rats were randomly divided into sham operation group,model group,valsartan group and SFI group according to random number table method,with 10 rats in each group.Ligation of left anterior descending coronary artery was performed to establish AMI model.The related drugs were administrated once a day for 14 days after model successful establishment.Echocardiography was applied to observe the changes of left ventricular wall motion and evaluate the cardiac function.Combining with hematoxylin-eosin staining,the morphological changes of myocardium were observed.Meanwhile,the proportion of type Ⅰ and Ⅲ collagen around marginal area of infarction was investigated by Sirius red staining.Immunohistochemical staining(IHC)combined with reverse transcription polymerase chain reaction(RT-PCR)were applied to detect the expression levels of myocardial apoptosis factor for Bcl-2 related X protein(Bax),B-cell lymphoma-2(Bcl-2)and cysteinyl aspartate specific proteinase-3(Caspase-3).Results The left ventricular ejection fraction(LVEF)and fractional shortening(LVFS)were significantly decreased in AMI group(P<0.01).Besides,the anterior wall of the left ventricle became thinner and its fluctuation weakened.In addition,the cardiac cavity was obviously dilated.The morphological analysis showed that there were few myocardial residual cells and high proportion of type Ⅰ and Ⅲ collagen with myocardial structure was seriously damaged in AMI rats(P<0.01).The expression levels of Bax/Bcl-2 and Caspase-3 mRNA in myocardium were significantly increased(P<0.01).SFI could essentially improve the cardiac structure of AMI rats,increase the thickness and movement of left ventricular wall,promote the levels of LVEF and LVFS.Moreover,it could reduce the proportion of type Ⅰ and Ⅲ collagen in marginal area of infarction while down-regulating the exp
