通塞脉片联合黄柏液外敷对气阴两虚兼血瘀型糖尿病足患者凝血功能及炎症指标的影响研究

摘　　要：目的探讨通塞脉片联合黄柏液外敷对气阴两虚兼血瘀型糖尿病足患者凝血功能及炎症指标的影响。方法将102例气阴两虚兼血瘀型糖尿病足患者随机分为对照组和试验组,每组51例。两组患者均予以常规基础疗法。对照组给予复方黄柏液涂剂外敷,每日1次;试验组在对照组基础上予以通塞脉片1.4 g口服,每日3次。两组均治疗4周。比较两组间临床疗效、凝血功能指标水平、炎症指标水平,以及生活质量和疼痛程度评分。结果对照组总有效率(78.43%,40/51)低于试验组总有效率(94.12%,48/51),差异有统计学意义(P<0.05)。与对照组比较,试验组治疗后凝血酶原时间(PT)、活化部分凝血酶原时间(APTT)水平较高,纤维蛋白原(FIB)水平较低,治疗后血清C反应蛋白(CRP)、白细胞计数(WBC)及中性粒细胞百分比(NEU%)水平较低,治疗后生活质量综合评定问卷(GQOL-74)评分较高,糖尿病患者生存质量特异性量表(DSQL)及视觉模拟评分法(VAS)评分较低,差异均有统计学意义(均P<0.05)。结论通塞脉片联合黄柏液外敷治疗气阴两虚兼血瘀型糖尿病足患者的疗效确切,能有效改善凝血功能,抑制炎症反应,减轻疼痛症状,提高患者生活质量。Objective To investigate the effect of Tongsaimai Tablet(通塞脉片)combined with Huangbai Liquid(黄柏液)for external application on coagulation function and inflammatory indexes in diabetes foot patients with deficiency of both Qi and Yin and blood stasis.Methods A total of 102 diabetes foot patients with deficiency of both Qi and Yin and blood stasis were randomly divided into control group and treatment group,each with 51 cases.The two groups were both given routine basic therapy.The control group was given Huangbai Liquid by external application,once a day.On the basis of the control group,the treatment group was given Tongsaimai Tablet 1.4 g orally,3 times a day.Two groups were both treated for 4 weeks.The clinical efficacy,coagulation function index level,inflammation index level,quality of life and pain score were compared between two groups.Results The total effective rate in the control group(78.43%,40/51)was lower than that in the treatment group(94.12%,48/51),and the differences were statistically significant(P<0.05).Compared with those in the control group,the levels of prothrombin time(PT)and activated partial prothrombin time(APTT)were higher,and the levels of fibrinogen(FIB)were lower.The levels of serum C-reactive protein(CRP),white blood cell count(WBC)and percentage of neutrophils(NEU%)were lower after treatment,and the scores of the Generic Quality of Life Inventory-74(GQOL-74)were higher and the scores of Diabetes Specific Quality of Life Scale(DSQL)and visual analogue scale(VAS)in patients with diabetes were lower,and the differences were all statistically significant(all P<0.05).Conclusion Tongsaimai Tablet combined with Huangbai Liquid in the treatment of diabetes foot patients with deficiency of both Qi and Yin and blood stasis has definite curative effect,which can effectively improve the coagulation function,inhibit the inflammatory reaction,reduce the pain symptoms and improve the quality of life of patients.
