摘 要:目的探讨针刺对老年小鼠术后认知功能障碍(POCD)的改善效果以及对载脂蛋白E(ApoE)、突触后致密物95(PSD-95)表达的影响。方法将80只小鼠随机均分为对照组、假手术组、POCD组和针刺组,每组20只,POCD组和针刺组小鼠经腹腔探查+阑尾切除+脾切除术制成POCD模型,假手术组小鼠只做切口不做手术,对照组不做任何处理,针刺组术后次日进行百会、手三里、曲池、足三里和昆仑穴位电针刺激,2次/d,10 min/次,连续21 d。Y迷宫实验检测各组小鼠行为学能力,透射电镜观察小鼠海马突触超微结构变化,免疫印迹法检测小鼠海马ApoE、PSD-95、Tau、p38丝裂原活化蛋白激酶(p38 MAPK)和β淀粉样蛋白(Aβ)蛋白表达,反转录多聚酶链反应(RT-PCR)检测小鼠海马突触素(SYP)、PSD-95 mRNA水平。结果与对照组、假手术组相比,POCD组和针刺组小鼠错误反应次数(EN)、单次到达安全区时间、全天总反应时间(TRT)和海马ApoE、Tau、磷酸化p38 MAPK(p-p38 MAPK)/p38 MAPK、Aβ蛋白表达均升高,而海马区突触密度、PSD-95蛋白表达和SYP、PSD-95 mRNA水平均低于对照组、假手术组(P<0.05)。与POCD组相比,针刺组小鼠EN、单次到达安全区时间和海马TRT、ApoE、Tau、p-p38 MAPK/p38 MAPK和Aβ蛋白表达均降低(P<0.05),而海马区突触密度、PSD-95蛋白表达和SYP、PSD-95 mRNA水平均升高(P<0.05)。结论针刺对老年小鼠POCD的行为学能力和海马突触微结构损伤有改善作用,可能与调节突触可塑性相关蛋白表达水平和抑制p38 MAPK活性有关。Objective To explore the improvement effect of acupuncture on postoperative cognitive dysfunction(POCD)in aged mice and its effects on apolipoprotein E(ApoE)and postsynaptic density 95(PSD-95).Methods A total of 80 mice were randomly divided into control group,sham operation group,POCD group and acupuncture group,20 cases in each group.In POCD group and acupuncture group,all underwent abdominal exploration,appendectomy and splenectomy to prepare POCD models.In sham operation group,there was only incision and no surgery.In control group,there was no any treatment.In acupuncture group,electroacupuncture stimulation at Baihui(DU20),Shousanli(LI10),Quchi(LI11),Zusanli(ST36)and Kunlun(BL60)was conducted on the next day after surgery(twice a day,10 min/once,21 d).The behavioral ability of mice was detected by Y-maze test.The ultrastructural changes of hippocampal synapses were observed by transmission electron microscopy.The expressions of ApoE,PSD-95,Tau,p38 mitogen-activated protein kinase(p38 MAPK)and β-amyloid(Aβ)in hippocampus were detected by Western blot.The levels of synaptophysin(SYP)and PSD-95 mRNA in hippocampus were detected by reverse transcription polymerase chain reaction(RT-PCR).Results Compared with those of the control group and sham operation group,error number(EN),once arrival time to safe zone,total reaction time(TRT)and expressions of ApoE,Tau,phosphory lation p38 MAPK(p-p38 MAPK)/p38 MAPK and Aβ in hippocampus were increased in POCD group and acupuncture group,while synaptic density,expression of PSD-95 protein and levels of SYP and PSD-95 mRNA in hippocampus were decreased(P<0.05).Compared with those of the POCD group,EN,once arrival time to safe zone,TRT and expressions of ApoE,Tau,p-p38 MAPK/p38 MAPK and Aβ in hippocampus were decreased in acupuncture group,while synaptic density,expression of PSD-95 protein and levels of SYP and PSD-95 mRNA in hippocampus were increased(P<0.05).Conclusion Acupuncture can improve behavioral ability and microstructure damage of hippocampal synapsis in aged
