摘  要:目的 分析儿童呼吸道合胞病毒感染严重程度与炎症指标、微小核糖核酸-146a(miR-146a)表达水平及基因分型的关系。方法 选取2021年9月-2022年9月杭州市临平区妇幼保健院收治的262例呼吸道合胞病毒感染患儿作为合胞病毒感染组,选取同期在医院进行检查的280名健康儿童作为健康对照组,比较两组炎症指标、miR-146a表达水平及不同病情严重程度呼吸道合胞病毒感染患儿炎症指标、miR-146a表达水平、基因型分布,采用Spearman相关系数分析呼吸道合胞病毒感染患儿病情严重程度与炎症指标、miR-146a表达水平基因型的相关性。结果 合胞病毒感染组血清干扰素λ1(IFN-λ1)、单核细胞趋化蛋白-1(MCP-1)、干扰素诱生蛋白10(IP-10)水平及miR-146a表达水平高于健康对照组(P<0.05);轻度组、中度组、重度组血清IFN-λ1、MCP-1、IP-10及miR-146a表达水平呈逐渐升高趋势(P<0.05);轻度组、中度组、重度组A亚型占比呈升高趋势,B亚型占比呈降低趋势(P<0.05);Spearman相关分析结果显示,血清IFN-λ1、MCP-1、IP-10水平、miR-146a表达水平、A亚型基因型与呼吸道合胞病毒感染患儿病情严重程度呈正相关关系(P<0.05),B亚型基因型与呼吸道合胞病毒感染患儿病情严重程度呈负相关关系(P<0.05)。结论 随着呼吸道合胞病毒感染患儿病情严重程度加重,其血清IFN-λ1、MCP-1、IP-10水平、miR-146a表达水平及A亚型基因型占比升高,B亚型基因型占比降低,炎症指标、miR-146a表达水平、基因型分布与呼吸道合胞病毒感染患儿病情严重程度相关。OBJECTIVE To analyze the relationship between the severity of respiratory syncytial virus infection and inflammatory markers,the expression level of micro ribonucleic acid(miR)-146a and genotyping in children.METHODS Total of 262 children with respiratory syncytial virus infection admitted to Hangzhou Linping District Maternal and Child Health Hospital from Sep 2021 to Sep 2022 were recruited in the syncytial virus infection group,and 280 healthy children under physical examination in our hospital during the same period were enrolled as the healthy control group.Inflammation indexes and expression level of miR-146a between the two groups were compared and inflammatory indexes,expression level of miR-146a and genotype distribution of children with respiratory syncytial virus infection among children with different severity were compared.Spearman correlation coefficient was used to analyze the correlation between the severity of respiratory syncytial virus infection and inflammation index and the expression level of miR-146a genotype.RESULTS The serum levels of interferon X1(IFN-X1),monocyte chemoattractant protein-1(MCP-1),interferon induced-protein 10(IP-10)and miR-146a in the syncytic virus infection group were higher than those in the healthy control group(P<0.05).Serum levels of IFN-X1,MCP-1,IP-10 and miR-146a among the mild group,the moderate group and the severe group gradually increased(P<0.05).The proportion of subtype A among the mild group,the moderate group and the severe group showed an trend of increase,while the proportion of subtype B showed a trend of decrease(P<o.05).Spearman correlation analysis showed that serum levels of IFN-αl,MCP-1 and IP-10,miR-146a expression and genotype A were positively correlated with the severity of infection with respiratory syncytic virus in children(P<0.05).There was a negative correlation between subtype B genotype and the severity of respiratory syncytial virus infection(P<0.05).CONCLUSION With the aggravation of respiratory syncytial virus infection,the serum le
