流感病毒抗原检测的PDCA管理实践及其效果

摘　　要：目的 运用计划-执行-检查-处理(PDCA)管理模式完善流感病毒抗原检测工作,持续提升流感病毒抗原检测操作质量,提高流感病毒抗原检测阳性检出率。方法 随机选取2022年8月16日-2022年8月30日314例符合流感样症状和体征的患者为研究对象,以提高流感病毒抗原检测阳性检出率为目的,运用PDCA管理模式完善管理制度,加强护理人员流感病毒检测教学培训,确保护理人员流感病毒抗原检测操作标准统一,做到流感病毒抗原检测标准化。分别从患者一般情况(性别、年龄)、客观结果(体温、白细胞计数)对比PDCA管理前后流感病毒抗原检测阳性检出率的变化。结果 运用PDCA管理模式后,护理人员流感病毒抗原检测合格率为94.90%,高于PDCA管理模式前69.11%(P<0.05);流感病毒抗原阳性检出率为23.89%,高于PDCA管理模式前12.42%(P<0.05);依照性别分组运用PDCA管理模式后男性、女性阳性检出率高于PDCA管理模式前(P<0.05);依照年龄分组运用PDCA管理模式后≥60岁、<60岁患者阳性阳性检出率高于PDCA管理模式前(P<0.05);依照体温分组运用PDCA管理模式后≥38.0℃、<38.0℃阳性检出率高于PDCA管理模式前(P<0.05);依照白细胞(WBC)分组WBC≥11×10^(9)/L、WBC<11×10^(9)/L阳性检出率高于PDCA管理模式前(P<0.05);PDCA管理模式后患者接受度为75.48%,高于PDCA管理模式前45.22%(P<0.05)。结论 运用PDCA管理模式,通过教学培训及考核制度,能够有效提升流感病毒抗原检测质量,提高流感病毒抗原检测阳性检出率,对院感防控保证诊疗质量具有重要意义,对临床工作起指导作用。OBJECTIVE To use plan-do-check-act(PDCA)management model to improve the process of influenza virus antigen detection,the quality of influenza virus antigen detection operations,and to improve the positive detection rate of influenza virus.METHODS During the period from Aug 16,2022 to Aug 30,2022,314 patients with influenza-like symptoms and signs were enrolled.In order to improve the positive detection rate of influenza virus antigen testing,the PDCA management model was used to strengthen the teaching and training of influenza virus testing in nursing staff,to ensure the unified operation standards and achieve standardization of influenza virus antigen testing.The patient's general situation(gender,age)and results of body temperature and white blood cell count were compared before and after PDCA management.RESULTS After PDCA management application,the qualified rate of influenza virus antigen detection in nursing staff was 94.90%,which was higher than that of 69.11%before PDCA management(P<0.05).The positive detection rate of influenza virus antigen was 23.89%,which was higher than that of 12.42%before PDCA management(P<0.05).The positive detection rates in male and female after PDCA management was higher than those before PDCA management(P<0.05).The positive detection rates in patients≥60 years old and<60 years old after PDCA management were higher than those before PDCA management(P<0.05).The positive detection rate in patients with the temperature≥38.0℃ and<38.0℃ after PDCA management was higher than those before PDCA management(P<0.05).The positive detection rates in patients with white blood cell(WBC)≥11 ×10^(9)/L and WBC<11 ×10^(9)/L were higher than those before PDCA management(P<0.05).The acceptance in patients after PDCA management was 75.48%,which was higher than 45.22%before PDCA management(P<0.05).CONCLUSION The application of PDCA management model by teaching,training and assessment system can effectively improve the quality of influenza virus antigen testing,improve the positive detecti
