强制运动疗法联合舍曲林对高血压脑出血偏瘫患者治疗依从性及上肢功能的影响

摘　　要：目的探讨强制运动疗法联合舍曲林对高血压脑出血偏瘫患者治疗依从性及上肢功能恢复的影响。方法将50例高血压脑出血偏瘫患者按照随机数字表法分为研究组与对照组,各25例。两组患者均行常规康复治疗和强制运动疗法治疗,研究组患者联合舍曲林治疗,观察6周。治疗前后采用上肢动作研究量表和Fugl-meyer上肢运动功能评分量表评定两组患者的上肢功能。比较两组患者退组率和意外事件。结果研究组患者的退组率(12%)低于对照组(44%),差异统计学意义(P<0.05)。治疗后,两组患者上肢动作研究量表及Fugl-meyer上肢运动功能评分量表评分均较治疗前升高,且研究组高于对照组,差异有统计学意义(P<0.05或0.01)。两组患者均无脑出血复发的病例出现。结论强制运动疗法治疗有助于改善高血压脑出血偏瘫患者的治疗依从性,促进患者上肢功能恢复,安全性高。Objective To explore the effect of constraint induced movement therapy(CIMT)combined with sertraline on treatment compliance and functional recovery of upper limbs in paralysis patients with hypertensive cerebral hemorrhage(HCH).Methods Fifty paralysis patients with HCH were assigned to research and control group according to random number table method,50 cases in each group.Both groups received routine rehabilitation treatment and the CIMT,the research was plus sertraline,and they were observed for 6 weeks.Upper limb functions were evaluated using the Action Research Arm Test(ARAT)and Fugl-Meyer Assessment Upper Extremity Scale(FMA-UE)before and after treatment.Dropout rates and accidents were compared between two groups.Results The dropout rate was lower in research than control group(12%vs 44%)and difference statistically significant(P<0.05).After treatment scores on the ARAT and FMA-UE elevated in both groups compared with pre-treatment,those were higher in research than control group,and differences statistically significant(P<0.05 or 0.01).There were no recurrent cases of cerebral hemorrhage in both groups.Conclusion CIMT plus-sertraline is conduce to the improvement of treatment compliance in paralysis patients with HCH,promote the recovery of upper limb functions,and has a high safety.
