摘  要:目的 幽门螺杆菌(Hp)在世界范围内感染非常普遍,Hp在胃中定植被确定为胃癌的最强危险因素,本研究旨在探讨Hp感染与白细胞计数之间的关联情况。方法 以2019年本院年龄≥18岁7 070例健康体检者为研究对象,回顾性分析其临床资料。采用13C尿素呼气试验用于检测Hp。收集包括人口统计学、临床特征、血清生物标志物等资料。应用Logistic回归探讨Hp感染与白细胞计数水平的相关性。结果 Hp感染率为36.80%。根据体检者白细胞(WBC)计数进行四等分组。Hp阳性者的WBC计数显着高于Hp阴性者(P<0.01)。Logistic回归分析显示,WBC计数水平在最高四分位数与最低四分位数相比,Hp感染率增加两倍(调整后OR=2.00,95%CI:1.71、2.33)。Hp感染和WBC计数感染之间呈现非线性关系,其拐点为6.602×10^(9)/L(左侧调整后OR=1.29,95%CI:1.19,1.40和右侧调整后OR=1.07,95%CI:0.96,1.18)。结论 Hp感染与白细胞增加呈正相关,在WBC小于6.602×10^(9)/L时,WBC计数增加每1.00×10^(9)/L个单位,Hp感染率增加29%。Objective Helicobacter pylori(Hp)infection is very common worldwide,and Hp colonization of the stomach has been identified as the strongest risk factor for gastric cancer,and the aim of this study was to investigate the association between Hp infection and white blood cell counts.Methods The clinical data of 7070health check-ups aged≥18years in our hospital in 2019were retrospectively analyzed.The 13 C urea breath test was used for the detection of Hp.Data including demographics,clinical characteristics,and serum biomarkers were collected.Logistic regression was applied to explore the correlation between Hp infection and white blood cell count levels.Results Hp infection rate was 36.80%.Quadratic groupings were made based on the white blood cell(WBC)counts of the medical examiners.The WBC counts of Hp-positive individuals were significantly higher than those of Hp-negative individuals(P<0.01).Logistic re-gression analysis showed a two-fold increase in the prevalence of Hp infections in the highest quartile compared with the lowest quartile of WBC count levels(adjusted OR=2.00,95%CI:1.71,2.33).The relationship between Hp infection and WBC count infection showed a non-linear relationship with an inflection point of 6.602×10^(9)/L(left adjusted OR=1.29,95%CI:1.19,1.40and right adjusted OR=1.07,95%CI:0.96,1.18).Conclusion Hp infection was positively correla-ted with increased leukocytes,with a 29%increase in Hp infection for every 1.00×10^(9)/L unit increase in WBC count at WBC less than 6.602×10^(9)/L.
