摘  要:目的比较自我血糖监测(SMBG)与连续动态血糖监测(CGM)在评估气阴两虚型2型糖尿病患者血糖控制方面的差异。方法收集2022年7月至2023年2月于上海宝山区中西医结合医院内分泌科住院的气阴两虚型2型糖尿病患者66例,分为预估糖化血红蛋白(eHbA1c)≥7%组51例及eHbA1c<7%组15例。两组均佩戴硅基动感动态血糖仪,并使用强生稳悦智优血糖仪检测指尖血糖。读取监测报告中24 h葡萄糖目标范围内时间(TIR)、24 h葡萄糖高于目标范围时间(TAR)、24 h葡萄糖低于目标范围内时间(TBR)和平均血糖(MBG)。以相同的血糖控制目标,计算SMBG所测得的指尖血糖TIR、TAR、TBR及MBG,比较SMBG与动态血糖监测在上述指标中的差异。同时分析葡萄糖标准差(SD)、变异系数(CV)、TIR的相关因素。结果两组采用SMBG与CGM,在MBG、TAR、TBR方面比较,差异有统计学意义(P<0.05),其中eHbA1c<7%组在TIR方面比较,差异无统计学意义(P>0.05)。eHbA1c≥7%组在TIR方面比较,差异有统计学意义(P<0.05)。SD与体重指数(BMI)、三酰甘油(TG)呈负相关,差异有统计学意义(P<0.05);CV与TG呈负相关,差异有统计学意义(P<0.05);TIR与BMI呈正关性,差异有统计学意义(P<0.05)。结论当e Hb A1c<7%时,SMBG和CGM两种方法在评价TIR方面一致。血糖波动与BMI、TG有一定的相关性。Objective To compare the difference between self-monitoring of blood glucose(SMBG)and continuous glucose monitoring(CGM)in evaluating blood glucose control in type 2 diabetes patients with deficiency of both qi and yin.Methods A total of 66 patients with type 2 diabetes of deficiency of both qi and yin type hospitalized in the Endocrinology Department of Shanghai Baoshan Hospital of Integrated Traditional Chinese and Western Medicine from July 2022 to February 2023 were collected and divided into an estimated glycated hemoglobin(eHbA1c)≥7%group(51 patients)and an eHbA1c<7%group(15 patients).Both groups wore silicon-based dynamic glucose meters and used the OneTouch VerioVue®meter to detect fingertip blood glucose.Monitoring reports were read for Time in Range(TIR),Time Above Range(TAR),Time Below Range(TBR),and Mean Blood Glucose(MBG)for 24 h glucose levels.TIR,TAR,TBR,and MBG of fingertip blood glucose measured by SMBG were calculated with the same blood glucose control goal,and the differences between SMBG and dynamic blood glucose monitoring in the above indicators were compared.At the same time,the correlation factors of glucose standard deviation(SD),coefficient of variation(CV)and TIR were analyzed.Results There are statistically significant differences in MBG,TAR,and TBR in the two groups using SMBG and CGM(P<0.05).Among them,the eHbA1c<7%group showed no statistically significant difference in TIR(P>0.05),while the eHbA1c≥7%group showed a statistically significant difference in TIR(P<0.05);The negative correlation between SD,Body Mass Index(BMI)and triglyceride(TG)was statistically significant(P<0.05);The negative correlation between CV and TG was statistically significant(P<0.05);The positive correlation between TIR and BMI was statistically significant(P<0.05).Conclusion When eHbA1c<7%,both SMBG and CGM methods are consistent in evaluating TIR.There is a certain correlation between blood glucose fluctuations and BMI and TG.
