处方前置审核在促进门急诊合理使用糖皮质激素的应用

摘　　要：目的促进糖皮质激素在临床的合理用药,保障患者用药安全,促进门急诊处方前置审核工作的开展。方法通过制订审方规则,结合医院用药实际情况,对糖皮质激素用药规则在适应证、用法用量、疗程、禁忌证、相互作用等方面进行精细化设置,以保障用药合理、安全、有效。选取湖南省娄底市中心医院门急诊药房未采用处方前置审核系统前(2021年6—10月)门急诊处方及使用处方前置审核系统后(2022年6—10月)门急诊处方,统计并分析处方前置审核前后急诊患者糖皮质激素静脉输液使用率和门急诊糖皮质激素处方合理率。结果对糖皮质激素进行处方前置审核后,急诊患者糖皮质激素静脉输液使用率显著下降,门急诊糖皮质激素处方合理率显著提高。结论糖皮质激素审方规则的合理设置有助于降低急诊患者糖皮质激素静脉使用率,提高门急诊糖皮质激素处方合理率,可促进临床合理用药,并培养了药师的综合能力。Objective To promote the rational medication of glucocorticoid in clinic practice,ensure the medication safety of patients,and promote the prescription pre-audit in outpatient and emergency departments.Methods By formulating the prescription audit rules and combining with the actual situation of medication in the hospital,the medication rules of glucocorticoid were carefully set in the aspects of indications,usage and dosage,course of treatment,contraindications and interaction,so as to ensure rational,safe and effective medication.The outpatient and emergency prescriptions of Loudi Central Hospital in Hunan Province before the adoption of the prescription pre-audit system(from June to October 2021)and after the adoption of the prescription pre-audit system(from June to October 2022)were selected.Meanwhile,the utilization rate of glucocorticoid intravenous infusion in emergency patients and the rational rate of glucocorticoid prescriptions in outpatient and emergency departments before and after the prescription pre-audit were statistically analyzed.Results After the prescription pre-audit of glucocorticoid,the utilization rate of intravenous infusion of glucocorticoid in emergency patients decreased significantly,and the rational rate of glucocorticoid prescription in outpatient and emergency departments increased significantly.Conclusion The reasonable setting of glucocorticoid prescription audit rules is helpful to reduce the intravenous utilization rate of glucocorticoid in emergency patients,improve the rational rate of glucocorticoid prescription in outpatient and emergency departments,promote clinical rational drug use,and cultivate the comprehensive ability of pharmacists.
