摘 要:目的:探究室上性心动过速(简称室上速)射频消融术后患者N末端脑钠肽前体(NT-proBNP)水平变化。方法:选取确诊的40例室上性心动过速患者为室上速组,另选取40例同期正常体检者为对照组,比较两组个体血清NT-proBNP水平差异,记录室上速组患者术前10 min、术后即刻、术后2 h以及术后24 h时NT-proBNP水平,比较不同射频消融位置(左侧组和右侧组)患者不同观察点NT-proBNP水平差异。结果:室上速组患者血清NT-proBNP平均水平明显高于对照组(P<0.05);相较于术前10 min、术后即刻、术后2 h和术后24 h左侧组及右侧组患者血清NT-proBNP水平均出现了明显的升高,前后差异具有统计学意义(P<0.05);左侧组和右侧组射频消融术的刺激强度和刺激时间比较差异无统计学意义(P>0.05),左侧组和右侧组患者在围术期不同观察点上,血清NT-proBNP水平组间差异无统计学意义(P>0.05)。结论:室上性心动过速患者血清NT-proBNP水平会出现异常升高,而射频消融术会进一步导致此类患者NT-proBNP水平出现上升,分析其原因可能与射频能量导致心肌细胞损伤和死亡,最终导致了B型利钠肽合成分泌增加有关。Objective To investigate the changes of N-terminal pro-brain natriuretic peptide(NT-proBNP)level in patients with supraventricular tachycardia after radiofrequency ablation.Method 40 patients with supraventricular tachycardia diagnosed in our hospital were selected as the supraventricular tachycardia group,and another 40 patients with normal physical examination in our hospital at the same time were selected as the control group.The difference of serum NT-proBNP level between the two groups was compared,and the NT-proBNP level of the patients in the supraventricular tachycardia group was recorded 10 minutes before operation,immediately after operation,2 hours after operation and 24 hours after operation,the difference of NT-proBNP level at different observation points in patients with different radiofrequency ablation positions(left group and right group)was compared.Results The average level of NT-proBNP in the patients with supraventricular tachycardia was significantly higher than that in the control group(P<0.05);Compared with 10 min before operation,the serum NT-proBNP level of patients in the left and right groups increased significantly immediately,2 h and 24 h after operation,and the difference was statistically significant(P<0.05);There was no significant difference in the comparison of stimulation intensity and stimulation time between the left and right groups for radiofrequency ablation(P>0.05).There was no significant difference in serum NT-proBNP level between the left and right groups at different observation points during the perioperative period(P>0.05).Conclusion The serum NT-proBNP level in patients with supraventricular tachycardia will increase abnormally,and radiofrequency ablation will further lead to the increase of NT-proBNP level in such patients.The reason may be related to the injury and death of cardiac myocytes caused by radiofrequency energy,which ultimately leads to the increase of B-type natriuretic peptide synthesis and secretion.
