摘 要:目前,肺移植手术是诸多类型终末期肺疾病的首选治疗方法,然而肺移植受者常因病程迁延多伴随严重的心肺功能障碍,其中多表现为呼吸衰竭及不同程度的肺动脉高压。体外膜肺氧合(ECMO)是一项体外支持呼吸、循环的有效手段,可过渡性支持危重患者的心肺功能,在肺移植过程中发挥着重要作用。本文就ECMO在肺移植手术中的应用价值、置管策略及管理、疗效评价等方面进行综述,以期加深ECMO在肺移植临床工作中的应用体会。Objective At present,lung transplantation is the preferred treatment for many types of end-stage lung diseases.However,these patients are often accompanied by serious cardiopulmonary dysfunction due to the prolonged course of disease,including respiratory failure and varying degrees of pulmonary hypertension.Lung transplantation is difficult,complicated to manage and has a high fatality rate.Extracorporeal membrane oxygenation(ECMO)is an effective method to support respiration and circulation in vitro,which can assist to maintain cardiopulmonary function in critically ill patients,and plays an important role in the process of lung transplantation.This article reviews the application value,intraoperative management and efficacy evaluation of ECMO in lung transplantation,so as to deepen the experience of extracorporeal membrane oxygenation in the clinical practice of lung transplantation.
