阿达木单抗致银屑病患者四肢水肿1例

摘　　要：报告银屑病患者使用阿达木单抗后诱发四肢水肿1例。患者男,60岁,因全身皮肤红斑、鳞屑10余年伴四肢水肿2周余就诊。患者既往确诊为银屑病,曾规律注射阿达木单抗治疗,注射第3针后患者皮损改善,但逐渐出现四肢凹陷性水肿。予利尿治疗1周后水肿仍未完全消退,按时注射第4针后四肢水肿加重。诊断该患者的四肢水肿为阿达木单抗诱发的罕见不良反应。予停用阿达木单抗,3周后水肿消退,随访3个月未复发。We report a case of extremity edema induced by adalimumab in a psoriatic patient.A 60-year-old man presented with erythema and scaling on the whole body for more than 10 years and edema on the extremities for over 2 weeks.The patient had been diagnosed with psoriasis and treated with adalimumab regularly.After the third injection of adalimumab,the skin lesions were improved,but the pitting edema was gradually developed on the extremities.After one week of diuretic treatment,the edema did not completely subside,but became worse after the fourth injection.Three weeks after discontinuation of adalimumab,edema subsided.No recurrence was observed during three-month follow-up.
