老年高血压患者血清sST 2、Hcy、Gal-3水平与射血分数保留性心力衰竭发生的相关性

摘　　要：目的探讨老年高血压患者血清可溶性肿瘤发生抑制蛋白2(sST2)、同型半胱氨酸(Hcy)、半乳糖凝集素3(Gal-3)水平与射血分数保留性心力衰竭(EFpHF)发生的相关性,以期为临床早期诊断、制定治疗方案提供依据。方法选取2020年5月至2022年10月南阳市第一人民医院收治的83例老年高血压患者为研究对象,其中单纯高血压患者51例(对照组),高血压合并EFpHF患者32例(研究组)。比较两组入院时血清sST2、Hcy、Gal-3水平,比较两组入院时超声心动图指标[左心房内径(LAD)、右心房内径(RAD)、二尖瓣口舒张早期最大血流速度E峰与舒张晚期最大血流速度A峰的比值(E/A)、左心室质量指数(LVMI)],分析入院时血清sST2、Hcy、Gal-3水平与超声心动图指标及EFpHF发生的相关性,受试者工作特征(ROC)曲线分析血清sST2、Hcy、Gal-3水平联合检测对老年高血压患者发生EFpHF的诊断价值。结果与对照组相比,入院时研究组血清sST2、Hcy、Gal-3水平较高(P<0.05);入院时与对照组相比,研究组LAD、RAD、LVMI指标明显升高,E/A指标明显降低(P<0.05);入院时血清sST2、Hcy、Gal-3水平与LAD、RAD、LVMI、EFpHF呈正相关,与E/A呈负相关(P<0.05);入院时血清sST2、Hcy、Gal-3水平联合检测诊断高血压患者发生EFpHF的曲线下面积(AUC)为0.841,大于各单一指标,最佳诊断敏感度为87.50%,特异度为80.39%。结论血清sST2、Hcy、Gal-3水平变化与老年高血压患者并发EFpHF密切相关,为临床早期诊断提供参考依据,以尽早制定相应干预方案,改善预后。【Objective】To investigate the correlation between serum soluble tumorigenesis suppressor protein 2(sST2),homocysteine(Hcy)and galactose lectin 3(Gal-3)levels and the occurrence of ejection fraction preserved heart failure(EFpHF)in elderly hypertensive patients,with a view to providing a basis for early clinical diagnosis and formulation of treatment plans.【Methods】Eighty-three elderly patients with hypertension admitted to our hospital from May 2020 to October 2022 were selected for the study,including 51 patients with hypertension alone(control group)and 32 patients with hypertension combined with EFpHF(study group).Serum sST2,Hcy,and Gal-3 levels at admission were compared between the two groups,and echocardiographic indices at admission[left atrial internal diameter(LAD),right atrial internal diameter(RAD),ratio of maximum early diastolic flow velocity E peak to maximum late diastolic flow velocity A peak(E/A)at the mitral valve orifice,and left ventricular mass index(LVMI)]were compared between the two groups,and serum sST2,Hcy,Gal-3 levels at admission were correlated with echocardiographic indices and the occurrence of EFpHF,and receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of combined serum sST2,Hcy,Gal-3 levels on the occurrence of EFpHF in elderly hypertensive patients.【Results】Compared with the control group,serum sST 2,Hcy,and Gal-3 levels were higher in the study group at admission(P<0.05).LAD,RAD,and LVMI indexes were significantly higher and E/A indexes were significantly lower in the study group compared with the control group at admission(P<0.05).Serum sST 2,Hcy,and Gal-3 levels at admission were positively correlated with LAD,RAD,LVMI and EFpHF,and negatively correlated with E/A(P<0.05).The area under curve(AUC)of the combined test of serum sST 2,Hcy,and Gal-3 levels at admission to diagnose the occurrence of EFpHF in hypertensive patients was 0.841,which was greater than each single index,with the best diagnostic sensitivity of 87.50%and specifi
