海花调肺方治疗早期肺癌术后伴肺部多发亚厘米磨玻璃结节疗效观察

摘　　要：目的观察海花调肺方(海浮石、旋覆花等)治疗早期肺癌术后伴肺部多发亚厘米磨玻璃结节的临床疗效。方法应用海花调肺方治疗60例早期肺癌术后伴肺部多发亚厘米磨玻璃结节患者,观察3个月和6个月患者肺结节CT征象(大小、密度、数量)和中医症状,评估临床疗效、结节恶性风险概率和中医证候疗效,监测安全性指标。结果60例患者术后初诊前肺结节变化情况为:肺结节0例缩小、31例稳定、14例进展,15例未复查。海花调肺方治疗3个月和6个月较治疗前目标肺结节的长径明显缩小(P<0.05),肺结节数量明显减少(P<0.05);治疗6个月与治疗3个月相比,目标结节长径缩小、密度减淡、目标结节吸收、结节数量减少的患者占比增加;治疗3个月和6个月的临床总有效率分别为35.00%(21/60)和51.67%(31/60),治疗6个月的总有效率优于治疗3个月(P<0.05);与治疗前比较,治疗3个月和6个月均可降低肺结节恶性风险概率(P<0.05),减轻中医证候和肺癌术后咳嗽症状(P<0.05),治疗3个月和6个月的中医证候总有效率分别为66.67%(40/60)和91.67%(55/60),治疗6个月的中医证候总有效率优于治疗3个月(P<0.05);治疗后安全性指标未见异常。结论海花调肺方治疗早期肺癌术后伴肺部多发亚厘米磨玻璃结节有良好的临床疗效,且安全性较好。Objective To observe the clinical effect of Haihua Tiaofei Recipe(bryozoatum,inulae Flos,etc.)in the treatment of patients with early-stage lung cancer after surgery accompanied by multiple sub-centimeter ground-glass nodules.Methods Haihua Tiaofei Recipe was used to treat 60 cases early-stage lung cancer after surgery accompanied by multiple sub-centimeter ground-glass nodules.The CT signs(size,density and quantity)of pulmonary nodules and traditional Chinese medicine(TCM)symptoms were observed after 3 and 6 months of treatment.The therapeutic efficacy,the probability of malignant for ground-glass opacity and the efficacy of TCM syndromes were evaluated,and the safety indicators were monitored.Results The changes in pulmonary nodules of 60 patients before initial diagnosis were as follows:0 patients for reduction of pulmonary nodules,31 patients of stable pulmonary nodules,14 patients of growing pulmonary nodules,and 15 non-reviewed patients.The diameter and the number of target pulmonary nodules decreased significantly after 3 and 6 months of treatment of Haihua Tiaofei Recipe(all P<0.05).Compared with 3months of treatment,the proportion of patients with reduction in diameter reduction and density of target nodules,nodular absorption,and number reduction of nodules after treatment of 6 months increased.The clinical efficiency of treatment in the period of 3 months and 6 months was 35.00%(21/60)and 51.67%(31/60),respectively,and the effective rate of treatment in the period of 6 months was better than that of 3 months(P<0.05).Compared with those before treatment,after treatment for 3 and 6 months,Haihua Tiaofei Recipe reduced the probability of malignant risk of pulmonary nodules(P<0.05),alleviated the TCM syndromes and cough symptoms after lung cancer surgery(P<0.05).The total effective rates of TCM syndromes of treatment in the period of 3 and 6 months were 66.67%(40/60)and 91.67%(55/60)respectively,and the effective rate of TCM syndromes of treatment in the period of6 months was better than that of 3 months(P
