摘  要:目的:探讨妊娠早期营养指标与新生儿出生体质量的关系。方法:本研究采用前瞻性观察性研究方法,选取2020年12月至2021年5月在青岛市妇女儿童医院营养科进行早孕营养评估的孕妇作为研究对象,收集孕妇的基本信息以及孕早期空腹血糖、血红蛋白、红细胞压积、白蛋白、转铁蛋白等实验室指标,并进行人体成分测定,随访至分娩共计收集研究对象1251例。比较分娩巨大儿与正常体质量儿两组孕妇的基础指标、实验室指标与体成分指标,采用Pearson相关及多因素逐步线性回归分析新生儿出生体质量的早期相关指标。结果:巨大儿发生率为8.5%,巨大儿组的孕前和孕早期BMI、孕早期血红蛋白、红细胞压积、空腹血糖以及体成分各指标均高于正常体质量儿组(P<0.05)。Pearson相关及多因素逐步线性回归分析显示孕早期空腹血糖和体内水分是影响新生儿出生体质量的主要因素(P<0.05)。结论:妊娠早期结合实验室和体成分检测进行早期营养评估,有助于预测新生儿出生体质量,应早期识别高危因素早期干预从而改善母婴预后。Objective:To investigate the relationship between early pregnancy nutrition index and birth weight of newborns.Methods:In this study,we used a prospective observational study method to select pregnant women who underwent early pregnancy nutritional assessment in the nutrition department of our hospital from December 2020 to May 2021 as study subjects.The basic information of pregnant women and laboratory indicators such as fasting glucose,hemoglobin,erythrocyte pressure product,albumin and transferrin in early pregnancy were collected,and body composition measurement were also performed.A total of 1251 study subjects from follow-up to delivery were enrolled.The basic indexes,laboratory indexes and body composition indexes of pregnant women who delivered giant babies and normal weight babies were compared.Pearson correlation and multi-factor stepwise linear regression were used to analyze the early related indexes of newborn birth weight.Results:The incidence of macrosomia was 8.5%.The indexes of pre-pregnancy and early pregnancy BMI,early pregnancy hemoglobin,hematopoietic volume,fasting blood glucose and body composition in giant baby group were higher than those in normal body weight group(P<0.05).Pearson correlation and multi-factor steppet-linear regression analysis showed that fasting blood glucose in early pregnancy and body water were the main factors affecting newborn birth weight(P<0.05).Conclusion:Early nutritional assessment in early pregnancy combined with laboratory and body composition detection is helpful to predict newborn birth weight.Early identification of high risk factors and early intervention should be carried out to improve maternal and infant prognosis.
