改良拱顶石皮瓣技术与Karydakis皮瓣术治疗骶尾部藏毛囊肿的疗效对比

摘　　要：目的比较改良拱顶石皮瓣技术(KSF)与Karydakis皮瓣术(下称Karydakis术)治疗骶尾部藏毛囊肿的疗效。方法选取2018年1月至2022年5月在宁波大学医学院附属医院肛肠外科行手术治疗的骶尾部藏毛囊肿患者54例为研究对象,按手术方式不同分为KSF皮瓣组20例和Karydakis皮瓣组34例,分别采用KSF术和Karydakis术治疗。比较两组患者手术时间、切除组织横径、住院时间、引流时间、愈合时间、恢复工作时间、并发症发生情况和复发情况。结果KSF皮瓣组患者手术时间、引流时间、愈合时间和恢复工作时间均短于Karydakis皮瓣组,差异均有统计学意义(均P<0.05)。KSF皮瓣组患者重度并发症发生率为10.0%,低于Karydakis皮瓣组的35.3%,差异有统计学意义(P<0.05)。两组患者均无一例复发。结论相较于Karydakis术,KSF术能缩短手术时间、愈合时间和恢复工作时间,还能降低伤口重度并发症发生率。Objective To compare the efficacy of surgical treatment with modified Keystone flap(KSF)and Karydakis flap for patients with sacrococcygeal pilonidal sinus.Methods The clinical data of 54 patients with sacrococcygeal pilonidal sinus treated in the Affiliated Hospital,Ningbo University School of Medicine from January 2018 to May 2022 were retrospectively analyzed,including 20 cases treated with KSF and 34 treated with Karydakis flap.The operation time,resected specimen diameter,length of hospital stay,drainage time,healing time,return to work time,local complications and recurrence were compared between the two groups.Results The operation time,drainage time,healing time and time to return to work in the KSF group were significantly shorter than those in the Karydakis group(all P<0.05).The incidence of severe complications was significantly lower in the KSF group than that in Karydakis group(10%vs.35.3%,P<0.05).There was no recurrence in two groups.Conclusion The KSF technique has the advantage of shortening the operative time and recovery time with less severe complications,compared to Karydakis flap in surgical treatment of patients with sacrococcygeal pilonidal sinus.
