摘 要:目的分析我国各地区骨科感染类疾病致病微生物的分布特点。方法检索万方数据、中国知网和维普自建库至2022年5月有关骨科感染类疾病致病微生物的文献,提取数据,分析导致该类感染的病原微生物的种类和耐药性特征。结果共纳入1997—2021年发表文献84篇,包含病例48533例,分离病原微生物28584株。其中,革兰阳性菌占47.19%,主要为金黄色葡萄球菌、表皮葡萄球菌和粪肠球菌;革兰阴性菌占51.31%,以铜绿假单胞菌、大肠埃希菌和阴沟肠杆菌为主;真菌仅占1.50%,计429株。耐甲氧西林金黄色葡萄球菌(methicillinresistant Staphylococcus aureus,MRSA)和耐甲氧西林凝固酶阴性葡萄球菌的检出率分别为50%和76%,MRSA的检出率在国内六大地理区域间比较差异有统计学意义。结论骨科感染的主要致病微生物为革兰阴性菌,金黄色葡萄球菌在所有致病菌中居于首位。目前,临床上针对凝固酶阴性葡萄球菌、真菌和病毒等的检测和治疗亟待提高。Objective To analyze the characteristics and distribution of pathogens isolated from orthopedic patients reported by various regions in China.Methods Literature on pathogenic microorganisms of orthopedic infectious diseases were retrieved from Wanfang data,CNKI and VIP from the self-built database to May 2022,and the data were extracted to analyze the types and drug resistance characteristics of pathogenic microorganisms causing such infections.Results A total of 84 articles published between 1997 and 2021 were selected,including 48533 cases and 28584 strains pathogenic microorganisms.Among them,Gram-positive bacteria accounted for 47.19%,mainly Staphylococcus aureus,Staphylococcus epidermidis and Enterococcus faecal.Gram-negative bacteria took on 51.31%with Pseudomonas aeruginosa,Escherichia coli and Enterobacter cloacae.There were only 429 strains of fungi,accounting for 1.50%.The detection rates of methicillin resistant Staphylococcus aureus(MRSA)and methicillin resistant coagulase-negative Staphylococcus were 50%and 76%,respectively,of which MRSA exhibited statistical significance among six major geographical regions.Conclusion Staphylococcus aureus ranks number one of all pathogens causing orthopedic infections,while Gram-negative bacteria are the most isolated from clinical specimens.The identification and treatment of coagulase-negative Staphylococcus,fungi and viruses are in need of improvement for clinical practice.
