腹腔镜辅助小切口肾部分切除术治疗复杂性肾癌的临床疗效

摘　　要：目的探讨腹腔镜辅助小切口肾部分切除术治疗复杂性肾癌的安全性及临床疗效。方法回顾性分析2020年6月至2022年6月于绍兴市中心医院就诊的68例复杂性肾癌患者的临床资料,根据手术方式的不同将患者分为观察组和对照组,每组各34例。观察组患者行腹腔镜辅助小切口肾部分切除术,对照组患者行传统开放肾部分切除术。比较两组患者的围手术期指标及并发症发生情况。结果两组患者手术均获得成功,均未出现切缘阳性,无中转根治性切除病例。观察组患者的并发症总发生率显著低于对照组(χ^(2)=2.527,P=0.032),住院时间、术后镇痛时间及胃肠道恢复时间均显著短于对照组(P<0.05)。结论腹腔镜辅助小切口肾部分切除术治疗复杂性肾癌安全可行,在保护肾功能的同时,具有微创、并发症少、术后恢复快的优点,可高效处理术中复杂情况。Objective To investigate the safety and clinical efficacy of laparoscopic assisted small incision partial nephrectomy in the treatment of complex renal carcinoma.Methods The clinical data of 68 patients with complex renal carcinoma treated in Shaoxing Central Hospital from June 2020 to June 2022 were retrospectively analyzed.According to different surgical methods,the patients were divided into observation group and control group,with 34 cases in each group.Observation group underwent laparoscopic assisted small incision partial nephrectomy,control group underwent traditional open partial nephrectomy.The perioperative indexes and complications were compared between the two groups.Results The operation was successful in both groups,and there was no positive margin and no conversion radical resection.The total complication rate of observation group was significantly lower than that of control group(χ^(2)=2.527,P=0.032).The hospitalization time,postoperative analgesia time and gastrointestinal recovery time of observation group were significantly shorter than those of control group(P<0.05).Conclusion Laparoscopic assisted small incision partial nephrectomy is safe and feasible in the treatment of complex renal carcinoma.It has the advantages of minimally invasive,fewer complications and quick postoperative recovery while protecting renal function,and can effectively deal with complex intraoperative conditions.
