摘　　要：为探讨胱抑素C(CysC)水平对维持性血液透析(MHD)患者发生心力衰竭(HF)的预测价值,选取2022年2月1日—2022年5月31日于南通大学第二附属医院肾内科行MHD的96例患者作为研究对象,收集患者的相关资料,根据随访12个月是否发生心衰,将其分为心衰组和非心衰组。比较两组患者检查指标,采用Logistic回归分析MHD患者心衰的独立危险因素;以ROC曲线评估CysC对MHD患者心衰的预测价值。结果显示,与非心衰组相比,心衰组患者CysC、β2-微球蛋白、白细胞水平显著升高(P<0.05),白蛋白水平显著降低(P<0.05);多因素Logisti回归分析显示,CysC升高是MHD患者心衰独立危险因素(P<0.05)。ROC曲线结果显示,CysC=6mg/L是预测MHD患者心衰的最佳临界点。研究发现,CysC是MHD患者发生心衰的独立危险因素,可作为MHD患者心衰的预测指标。Exploring the predictive value of cystatin C(CysC)levels for the occurrence of heart failure(HF)in maintenance hemodialysis(MHD)patients.96 patients who underwent MHD at the Department of Nephrology,The Second Affiliated Hospital of Nantong University from February 1,2022 to May 31,2022 were selected as the research subjects.Relevant data were collected from the patients,and they were divided into a heart failure group and a non heart failure group based on whether they had heart failure during a 12 month follow-up.Compare the examination indicators of two groups of patients and use logistic regression to analyze the independent risk factors of heart failure in MHD patients;Evaluate the predictive value of CysC for heart failure in MHD patients using ROC curves.The results showed that compared with the non heart failure group,CysCβThe levels of 2-microglobulin and white blood cells significantly increased(P<0.05),while the levels of albumin significantly decreased(P<0.05);Multivariate logistic regression analysis showed that elevated CysC was an independent risk factor for heart failure in MHD patients(P<0.05).The ROC curve results show that CysC=6mg/L is the optimal critical point for predicting heart failure in MHD patients.Research has found that CysC is an independent risk factor for heart failure in MHD patients and can serve as a predictive indicator for heart failure in MHD patients.
