摘 要:目的:探讨微柱凝胶技术应用于新生儿溶血试验诊断新生儿溶血病的临床价值。方法:选取2020年1—12月南京市高淳人民医院接受溶血试验的473例疑似新生儿溶血病的婴儿作为研究对象。采用微柱凝胶法测定新生儿ABO血型、RH (D)血型以及新生儿溶血试验指标(游离抗体、释放抗体、直接抗人球蛋白、间接抗人球蛋白),统计阳性率,观察不同血型新生儿溶血试验诊断结果,统计溶血病检出率以及试验检测结果分布。结果:473例新生儿中,A型血167例(35.31%)、B型血127例(26.85%)、O型血168例(35.52%)、AB型血11例(2.33%),RH (D)阳性473例(100.00%)。游离抗体阳性97例(20.51%)、释放抗体阳性156例(32.98%)、直接抗人球蛋白阳性6例(1.72%)、间接抗人球蛋白阳性2例(0.42%)。经统计,确诊新生儿溶血病156例,疾病检出率32.98%,其中A型血和B型血新生儿溶血病检出率分别为53.89%和53.28%,差异无统计学意义(P>0.05),均高于O型血(0.60%)和AB型血(0),差异有统计学意义(P<0.05)。结论:基于微柱凝胶技术进行新生儿溶血试验操作简单,敏感性高,可以有效检出新生儿溶血病,为临床诊治提供可靠的实验室依据。Objective:To investigate the clinical value of microcolumn gel technique for the diagnosis of neonatal hemolytic disease by neonatal hemolysis test.Methods:A total of 473 infants with suspected neonatal hemolytic disease who underwent hemolysis tests in hospitals from January to December 2020 were selected for the study.The microcolumn gel method was used to determine neonatal ABO blood group,RH(D) blood group,and neonatal hemolysis test indicators free antibody,released antibody,direct anti-human globulin,and indirect anti-human globulin.The positive rate was counted,and the diagnostic results of hemolysis test for newborns with different blood types were observed,and the detection rate of hemolytic disease and the distribution of test results were counted.Results:Of the 473 newborns,167(35.31%) were of blood type A,127(26.85%) of blood type B,168(35.52%) of blood type O,11(2.33%) of blood type AB,and 473(100.00%) were RH(D)-positive.97 cases(20.51%)were positive for free antibodies,156 cases(32.98%) were positive for released antibodies,6 cases(1.72%) were positive for direct anti-human globulin,and 2 cases(0.42%) were positive for indirect anti-human globulin.Statistically,156 cases of neonatal hemolytic disease were confirmed,with a disease detection rate of 32.98%,of which 53.89% and 53.28% were detected in neonates with blood type A and B,respectively,with no statistically significant difference(P>0.05),and were higher than those of blood type O(0.60%) and AB(0),and the difference was statistically significant(P<0.05).Conclusion:Based on the microcolumn gel technology,the neonatal hemolysis test is simple and sensitive,which can effectively detect neonatal hemolysis and provide a reliable laboratory basis for clinical diagnosis and treatment.
