子宫下段提拉式缝合术在再次剖宫产术中修补子宫疤痕缺陷中的应用

摘　　要：目的研讨在再次剖宫产术中修补子宫疤痕缺陷时应用子宫下段提拉式缝合术的效果。方法研究对象为河南科技大学附属许昌市中心医院2020年1月1日—2022年1月1日接诊的因疤痕子宫再次妊娠分娩采用剖宫产术,术中发现同时合并有子宫疤痕缺陷的产妇75例,数字表法随机分组,观察组(n=39)实施子宫下段提拉式缝合术,对照组(n=36)实施常规缝合,观察手术效果及术后恢复情况。结果除术中出血量,对照组产妇的手术时间、产后出血量及产后排恶露时间均显著大于观察组,并发症总发生率显著大于对照组(P<0.05)。结论子宫下段提拉式缝合术在再次剖宫产手术中可以很好的缝合子宫切口,同时也修补了前次手术造成的子宫疤痕缺陷,方法简单、实用,值得临床推广使用。Objective To study the effect of pulling suture of lower uterine segment in repairing uterine scar defect during cesarean section again.Methods The research object was cesarean section patients in the second pregnancy due to scar in uterus,admitted to our hospital from January 1,2020 to January 1,2022.During the operation,75 pregnant women complicate with scar defect in uterus were found,and there were randomly divided into groups by digital fable method the observation group(39 cases)were conducted,the lower uterine segment pulling suture,control group(36 cases)were conducted routine suture,and observed the surgical effect and postoperative recovery.Results Except intraoperative bleeding amun,the operation time postpartum bleeding amount are obviously more than that of observation group,complication occurance rate are higher than that of observation group.Conclusion Pulling up suture in the lower uterine segment can sew the uterine incision well in the second cesarean section,and repair the uterine scar defect caused by the previous operation.The method is simple and practical,and it is worthy of clinical application.
