糖尿病合并肺结核的胸部X线及CT诊断研究

摘　　要：目的:研究糖尿病合并肺结核的胸部X线及电子计算机断层扫描(CT)诊断。方法:选取2016年1月—2021年1月南平市第一医院150例疑似糖尿病合并肺结核患者,所有患者均给予胸部X线及CT检查,并以病理检查结果为“金标准”,对检查结果进行分析。结果:150例疑似糖尿病合并结核病患者病理确诊率80.67%;与CT诊断对比,胸部X线诊断灵敏度、特异度、符合率均较低,差异有统计学意义(χ^(2)=11.703、22.442、37.076,P<0.05);CT诊断淋巴结肿大、空洞、支气管播散灶、少量胸水、胸膜增厚、卫星灶检出率均高于胸部X线,差异有统计学意义(χ^(2)=4.624、21.540、7.545、22.581、5.684、20.056,P<0.05)。结论:对糖尿病合并肺结核患者给予CT诊断具有一定的灵敏度、特异度、符合率,对病灶病理特征诊断具有较高检出率,以此为临床诊断、治疗提供一定的理论依据。Objective:To study the chest X-ray and electron computed tomography(CT)diagnosis of diabetes mellitus combined with pulmonary tuberculosis.Methods:150 patients with suspected diabetes mellitus combined with pulmonary tuberculosis in hospitals from January 2016 to January 2021 were selected and all patients were given chest X-ray with CT scan.The pathological examination results were used as the“gold standard”to analyze the post-examination results.Results:The pathological confirmation rate of 150 patients with suspected diabetes mellitus combined with tuberculosis was 80.67%.Compared with CT diagnosis,the sensitivity,specificity and compliance rate of chest X-ray diagnosis were lower,and the difference was statistically significant(χ^(2)=11.703,22.442,37.076,P<0.05).CT diagnosis of lymph node enlargement,cavity,bronchial spread foci,small amount of pleural fluid,pleural thickening,and satellite foci were detected at a higher rate than chest X-ray,and the difference was statistically significant(χ^(2)=4.624,21.540,7.545,22.581,5.684,20.056,P<0.05).Conclusion:The CT diagnosis given to patients with diabetes mellitus combined with pulmonary tuberculosis has a certain sensitivity,specificity and compliance rate,and a high detection rate for the diagnosis of pathological features of the lesion,so as to provide a certain theoretical basis for clinical diagnosis and treatment.
