分级药学监护在心力衰竭患者中的实践与临床评价

摘　　要：目的:探讨药师为心衰患者提供分级药学监护的效果。方法:以苏州大学附属第一医院心衰患者为研究对象,结合心衰指南,制定分级药学监护标准。收集患者290例,随机分为干预组和对照组,根据分级标准给予患者不同级别(一级、二级和三级)的药学监护。分别在干预前、干预6个月后、干预12个月后,对2组患者的分级评分、心功能分级、心衰生活质量评分和用药依从性进行组间比较,观察不良反应发生情况以及临床有效治疗率。结果:与对照组相比,干预12个月后,干预组临床有效治疗率提高了21.50%,不良反应发生率降低了6.15%,一、二级监护人数显著降低,三级监护人数显著升高(P<0.01),心功能分级、心衰生活质量评分和用药依从性显著改善(P<0.01)。结论:为心衰患者提供分级药学监护,可以提高药学服务质量和临床效果。OBJECTIVE To explore the influence of pharmacists providing graded pharmaceutical care for patients with heart failure.METHODS Combined with guidelines on patients with heart failure,the standards for grading pharmaceutical care was established.Two hundrend and ninty patients were randomly divided into intervention group and control group.And patients were given different levels of pharmaceutical care(level 1,level 2 and level 3).Before intervention,6 months and 12 months after intervention,grading score,classification of heart function,quality of life score of heart failure and medication compliance were compared.And the rate of ADR and the clinical effective were observed.RESULTS Compared with control group,12 months after intervention,the clinical effective rate of intervention group was increased by 21.50%,the rate of ADR was reduced by 6.15%,the number of primary and secondary guardianship was significantly decreased,and the number of tertiary guardianship was significantly increased(P<0.01),classification of heart function,life quality score of heart failure and medication compliance was significantly improved(P<0.01).CONCLUSION Providing grading pharmaceutical care for patients with heart failure can improve the quality of pharmaceutical care and clinical effect.
