摘　　要：目的:探讨在信息化下建立个体化患者用药教育系统的方法和意义。方法:通过在院内建立一套患者教育系统,对接HIS系统里的门诊和出院患者的就诊信息,包括性别、年龄、诊断、妊娠、给药途径、用法用量、合用药品等,经系统规则运算后,推送出符合该患者自身特征的用药指导内容,初步实现个体化用药教育。结果:系统匹配了826种院内药品,分成25种药理大类,138种二级分类,建立特征分析1048条,规则集4395条;经统计,运行后日均推送8633条用药指导内容,可通过微信公众号、医院APP、出院带药单等途径接收,覆盖大部分就诊患者,起到很好的教育作用,同时内容可显示在发药界面供药师工作时参考。结论:利用患者教育系统,可以有效实现个体化用药指导,具有高效性、快速性、统一性,保证患者安全用药的同时,也减轻了药师工作量,为落实"医疗机构用药教育服务规范"政策提供了有力支撑。OBJECTIVE To explore the method and significance of establishing an individualized medication education system for patients by using information technology.METHODS A patient education system was set up in the hospital to connect with the information of discharged patients and outpatients in the Hospital Information System(HIS),including gender,age,diagnosis,pregnancy,route of administration,usage and dosage,concomitant drugs,and other information;after processing such information in accordance with the relevant rules,the medication guide that conformed to a patient's conditions and characteristics was provided,and the individualized medication education was realized preliminarily.RESULTS In the system,there were 826 kinds of drugs for the hospital,they were divided into 25 pharmacological categories,and 138 secondary classifications;a total of 1048 characteristics analyses and 4395 rule sets were established;according to the statistics,a total of 8633 medication guidance was pushed every day since the operation,and the guidance can be received through WeChat official account,hospital APP,discharge medication note and other channels,benefiting most patients,playing a very good role in education.In addition,the guidance could also be displayed on the drug distribution interface for pharmacists while working.CONCLUSION The patient education system can provide individualized medication guidance,with high efficiency,rapidness,and uniformity,to ensure safe medication,and reduce the workload of pharmacists.It also provides strong support for the implementation of the"Standard of Medication Education Service in Medical Institutions".
