外伤性颅脑损伤患者CT检查征象及Rotterdam CT评分与其病情程度的相关性研究

摘　　要：目的探讨外伤性颅脑损伤(TBI)患者CT检查征象,并分析颅脑外伤鹿特丹(Rotterdam)CT评分与其病情程度的相关性。方法选取2021年12月至2022年12月广州市番禺区第五人民医院医院收治的60例TBI患者作为研究对象,根据格拉斯哥昏迷评分(GCS)结果分为轻度组(12例)、中等组(21例)、重度组(27例),比较3组CT检查征象情况,并采用Spearman相关系数分析Rotterdam CT评分其与病情程度的相关性。结果3组基底池、中线移位、血肿、蛛网膜下腔出血、Rotterdam CT评分组间比较,差异有统计学意义(P<0.05);Spearman相关性分析显示:Rotterdam CT评分与其病情程度(赋值:轻度=1,中等=2,重度=3)呈正相关(r=0.72,P<0.001)。结论不同病情程度TBI患者呈现不同CT检查征象,Rotterdam CT评分与其病情程度关系密切,通过CT检查可为患者临床诊治提供指导。Objective To explore the CT signs of patients with traumatic brain injury(TBI),and analyze the correlation between the Rotterdam CT score and the severity of the disease.Methods Sixty patients with TBI admitted to the Fifth People's Hospital of Panyu District,Guangzhou from December 2021 to December 2022 were selected as the study subjects.According to the Glasgow coma scale(GCS)results,they were divided into mild group(n=12 cases),moderate group(n=21 cases),and severe group(n=27 cases).The CT signs of the three groups were compared,and the correlation between the Rotterdam CT score and the degree of illness was analyzed by Spearman correlation coefficient.Results There were statistically significant differences among the three groups in basal cistern,midline shift,hematoma,subarachnoid hemorrhage and Rotterdam CT score(P<0.005).Spearman correlation analysis showed that the Rotterdam CT score was positively correlated with the severity of the disease(assigned value:mild=1,moderate=2,severe=3)(r=0.72,P<0.001).Conclusion TBI patients with different degrees of disease present different CT signs.The Rotterdam CT score is closely related to the degree of disease.CT examination can provide guidance for clinical diagnosis and treatment of patients.
