通痹宁心汤联合常规西药治疗扩张型心肌病合并室性心律失常患者的效果

摘　　要：目的:观察通痹宁心汤联合常规西药治疗扩张型心肌病合并室性心律失常患者的效果。方法:选取2019年8月至2021年10月该院收治的94例扩张型心肌病合并室性心律失常患者进行前瞻性研究,按照随机数字表法分为研究组与对照组各47例。对照组采用常规西药治疗,研究组在对照组基础上联合通痹宁心汤治疗,比较两组疗效、血清内皮素-1(ET-1)水平、心率变异性指标[每隔5 min窦性心搏R-R间期平均值标准差(SDANN)、窦性心搏R-R间期标准差(SDNN)、相邻窦性心搏R-R间期差值均方根(RMSSD)]水平、QT间期离散度(QTd)和左心室射血分数(LVEF)水平。结果:研究组治疗总有效率为89.36%,明显高于对照组的72.34%,差异有统计学意义(P<0.05);治疗后,两组血清ET-1、QTd水平低于治疗前,且研究组低于对照组,两组SDNN、SDANN、RMSSD、LVEF水平高于治疗前,且研究组高于对照组,差异均有统计学意义(P<0.05)。结论:通痹宁心汤联合常规西药治疗扩张型心肌病合并室性心律失常患者可提高治疗总有效率和LVEF水平,降低其ET-1水平,改善其心率变异性和QTd水平,效果优于单纯常规西药治疗。Objective:To observe effects of Tongbi Ningxin decoction combined with conventional Western medicine in treatment of patients with dilated cardiomyopathy complicated with ventricular arrhythmia.Methods:A prospective study was conducted on 94 patients with dilated cardiomyopathy complicated with ventricular arrhythmia admitted to the hospital from August 2019 to October 2021.They were divided into study group and control group according to the random number table method,47 cases in each.The control group was treated with conventional Western medicine,while the study group was treated with Tongbi Ningxin decoction on the basis of that of the control group.The efficacy,the serum endothelin-1(ET-1)level,the heart rate variability index levels[standard deviation of mean R-R interval of sinus beats every 5 min(SDANN),standard deviation of R-R interval(SDNN),root mean square of difference of R-R interval between adjacent sinus beats(RMSSD)],the QT interval dispersion(QTd),and the left ventricular ejection fraction(LVEF)were compared between the two groups.Results:The total effective rate of the study group was 89.36%,which was significantly higher than 72.34%of the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of serum ET-1 and QTd in the two groups were lower than those before the treatment,and those in the study group were lower than those in the control group;the levels of SDNN,SDANN,RMSSD and LVEF in the two groups were higher than those before the treatment,and those in the study group were higher than those in the control group;and the differences were statistically significant(P<0.05).Conclusions:Tongbi Ningxin decoction combined with conventional Western medicine can improve the total effective rate and LVEF levels,reduce the ET-1 levels,and improve the heart rate variability and QTd levels in the patients with dilated cardiomyopathy complicated with ventricular arrhythmia.Moreover,it is superior to single conventional drug therapy.
