摘 要:目的:分析血管超声检查在椎动脉开口狭窄中的诊断效能。方法:选取2021年9月至2023年1月该院收治的189例疑似椎动脉开口狭窄患者进行前瞻性研究,所有患者均行血管超声检查,以数字减影血管造影(DSA)检查结果为“金标准”,并依据是否存在椎动脉开口狭窄分为狭窄组和非狭窄组;另选取同期80名健康体检者设为健康组,比较三组血管超声检查各项参数水平,并绘制受试者工作曲线(ROC)分析血管超声各项参数水平单项及联合检查诊断椎动脉开口狭窄的效能。结果:金标准结果显示,189例疑似椎动脉开口狭窄患者中椎动脉开口狭窄74例,检出率为39.15%(74/189),设为狭窄组;非椎动脉开口狭窄患者115例,设为非狭窄组;狭窄组收缩期峰值血流速度(PSV)、舒张末期血流速度(EDV)、搏动指数(PI)、阻力指数(RI)水平均高于非狭窄组、健康组,且非狭窄组高于健康组;狭窄组椎动脉内径水平低于非狭窄组、健康组,且非狭窄组低于健康组,差异有统计学意义(P<0.05);ROC曲线分析结果显示,PSV、EDV、椎动脉内径、PI、RI水平单项及联合检查诊断椎动脉开口狭窄的曲线下面积(AUC)分别为0.864、0.866、0.848、0.743、0.610、0.918,且联合检测诊断椎动脉开口狭窄的效能最高。结论:血管超声各参数水平联合检测诊断椎动脉开口狭窄的效能高于各参数水平单项检测诊断。Objective:To analyze diagnostic efficacy of vascular ultrasonography in vertebral artery ostial stenosis.Methods:A prospective study was conducted on 189 patients with suspected vertebral artery stenosis admitted to the hospital from September 2021 to January 2023.All patients underwent vascular ultrasonography.The results of digital subtraction angiography(DSA)were used as the“gold standard”,and the patients were divided into stenosis group and non-stenosis group according to whether there was vertebral artery ostial stenosis.Another 80 healthy subjects in the same period were selected as the healthy group.The levels of various parameters of vascular ultrasonography were compared among the three groups.The receiver operating curve(ROC)was drawn to analyze the efficacies of single and combined examination of vascular ultrasound parameters in the diagnosis of vertebral artery ostial stenosis.Results:The gold standard showed that among the 189 patients with suspected vertebral artery ostial stenosis,74 patients had vertebral artery ostial stenosis with a detection rate of 39.15%(74/189),which was set as the stenosis group;the patients with non-vertebral artery ostial stenosis were set as the non-stenosis group.The levels of peak systolic blood flow velocity(PSV),end-diastolic blood flow velocity(EDV),pulsation index(PI)and resistance index(RI)in the stenosis group were higher than those in the non-stenosis group and the healthy group,and those in the non-stenosis group were higher than those in the healthy group;the level of vertebral artery diameter in the stenosis group was lower than that in the non-stenosis group and the healthy group,and that in the non-stenosis group was lower than that in the healthy group;and the differences were statistically significant(P<0.05).The ROC showed that the areas under the curve(AUC)of PSV,EDV,vertebral artery diameter,PI and RI levels in the diagnosis of vertebral artery ostial stenosis were 0.864,0.866,0.848,0.743,0.610 and 0.918,separately,and the combined detection had
