多层螺旋CT联合CA19-9、CEACAM1、CA242检测在胰腺癌诊断中的效能

摘　　要：目的:探讨多层螺旋CT(MSCT)联合血清糖类抗原19-9(CA19-9)、糖类抗原242(CA242)、癌胚抗原相关细胞黏附分子1(CEACAM1)检测在胰腺癌诊断中的效能。方法:选取2019年1月至2020年12月该院收治的107例疑似胰腺癌患者为研究对象,所有患者均行MSCT扫描,并检测血清CA19-9、CEACAM1、CA242水平,以病理结果为金标准,绘制受试者工作特征曲线(ROC),分析MSCT联合血清CA19-9、CA242、CEACAM1检测在胰腺癌诊断中的效能。结果:病理结果显示,胰腺癌45例,占42.06%;胰腺良性病变62例,占57.94%。两组门脉期、胰腺期ΔCT值比较,差异无统计学意义(P>0.05);胰腺癌组动脉期ΔCT值大于胰腺良性病变组,差异有统计学意义(P<0.05)。胰腺癌组CA19-9、CA242、CEACAM1水平均高于胰腺良性病变组,差异有统计学意义(P<0.05)。ROC曲线分析结果显示,MSCT(动脉期ΔCT值)、CA19-9、CA242、CEACAM1单项及联合检测诊断胰腺癌的曲线下面积依次为0.743、0.842、0.764、0.861、0.932,联合检测诊断胰腺癌的效能高于各单项检测。结论:MSCT、血清CA19-9、CA242、CEACAM1联合检测诊断胰腺癌的效能高于各单项检测。Objective:To investigate efficacy of multi-slice spiral CT(MSCT)combined with serum carbohydrate antigen 19-9(CA19-9),carbohydrate antigen 242(CA242)and carcinoembryonic antigen-related cell adhesion molecule 1(CEACAM1)in diagnosis of pancreatic cancer.Methods:A total of 107 patients with suspected pancreatic cancer admitted to the hospital from January 2019 to December 2020 were selected as the research objects.All patients underwent MSCT scan.The levels of serum CA19-9,CEACAM1 and CA242 were detected.The pathological results were used as the gold standard.The receiver operating characteristic curve(ROC)was drawn to analyze the efficacy of MSCT combined with the serum CA19-9,CA242 and CEACAM1 detection in the diagnosis of pancreatic cancer.Results:The pathological results showed that 45 cases(42.06%)were suffered from pancreatic cancer;there were 62 cases of benign pancreatic lesions,accounting for 57.94%.There were no statistical differences in the portal and pancreatic phasesΔCT values between the two groups(P>0.05).There was no statistically significant difference in CT values(P>0.05).The arterial phaseΔCT value of the pancreatic cancer group were higher than that of the benign pancreatic lesion group,and the difference was statistically significant(P<0.05).The levels of CA19-9,CA242 and CEACAM1 in the pancreatic cancer group were significantly higher than those in the benign pancreatic lesion group,and the differences were statistically significant(P<0.05).The ROC analysis showed that the AUCs of MSCT(arterial phaseΔCT value),CA19-9,CA242,CEACAM1 single and combined detection in the diagnosis of pancreatic cancer were 0.743,0.842,0.764,0.861 and 0.932,separately.The efficacy of combined detection in the diagnosis of pancreatic cancer was higher than that of single detection.Conclusions:The combined detection of MSCT,serum CA19-9,CA242 and CEACAM1 in the diagnosis of pancreatic cancer is more effective than single detection.
