银屑病关节炎骨质破坏骨代谢标记物的筛查与研究

摘　　要：目的 研究骨代谢标记物在银屑病关节炎(PsA)骨质破坏中的特点,分析骨代谢标记物与PsA骨质破坏的内在关系。方法 选取新疆医科大学第六附属医院2022年1~10月收治的20例PsA骨质破坏患者为PsA骨质破坏组,选择同期20例银屑病(Ps)患者为Ps组,并纳入同期20例健康志愿者为健康组。比较PsA骨质破坏组及Ps组治疗前、治疗2个月、治疗3个月时与健康组体检时红细胞沉降率(ESR)、C-反应蛋白(CRP)及骨代谢指标[抗酒石酸酸性磷酸酶5b(TRACP5b)、Ⅰ型胶原C端肽(CTX-Ⅰ)、骨特异性碱性磷酸酶(BALP)]水平,同时分析ESR、CRP、TRACP5b、CTX-I、BALP指标与骨质破坏的相关性。结果 治疗前,PsA骨质破坏组ESR、CRP、TRACP5b、CTX-I、BALP水平均高于Ps组及健康组,差异均有统计学意义(P<0.05);PsA骨质破坏组治疗2个月、3个月时ESR、CRP、TRACP5b、CTX-I、BALP水平均高于Ps组,差异均有统计学意义(P<0.05);经点二列相关性分析显示,ESR、CRP、TRACP5b、CTX-I、BALP水平与骨质破坏呈正相关(P<0.05)。结论 ESR、CRP、TRACP5b、CTX-I、BALP在PsA骨质破坏中呈高表达水平,且其表达水平与骨质破坏发生呈正相关。Objective To observe the characteristics of bone metabolic markers in the bone destruction of psoriatic arthritis(PsA),and to analyze the internal relationship between bone metabolic markers and bone destruction in PsA.Methods A total of 20 PsA patients with bone destruction admitted to the Sixth Affiliated Hospital of Xinjiang Medical University from January to October 2022 were selected as the PsA bone destruction group.During the same period,20 patients with psoriasis(Ps),and 20 healthy volunteers were included in the Ps group and healthy group,respectively.The levels of erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)and bone metabolism indexes,including tartrate-resistant acid phosphatase 5b(TRACP5b),typeⅠcollagen C-terminal peptide(CTX-Ⅰ),and bone-specific alkaline phosphatase(BALP)before treatment,and 2 months and 3 months after treatment were compared between PsA and Ps group.Their correlations with bone destruction in PsA were identified.Results Before treatment,ESR,CRP,TRACP5b,CTX-Ⅰand BALP in PsA bone destruction group were significantly higher than those in Ps group and healthy group(P<0.05).ESR,CRP,TRACP5b,CTX-I and BALP at 2 and 3 months after treatment in PsA bone destruction group were significantly higher than those in Ps group(P<0.05).Point binary correlation analysis showed that ESR,CRP,TRACP5b,CTX-I,and BALP levels were positively correlated with bone destruction(P<0.05).Conclusion ESR,CRP,TRACP5b,CTX-I and BALP are highly expressed in PsA patients with bone destruction,and their expression levels are positively correlated with the occurrence of bone destruction.
