摘　　要：文章介绍著名呼吸病专家周平安教授在临床中运用益气解毒法治疗肺痹咳嗽之临证经验。周平安教授认为特发性肺纤维化(Idiopathic pulmonary fibrosis,IPF)咳嗽根据其证候特点在中医学概念中应命名为“肺痹咳嗽”,而在辨证治疗过程中须重视肺痹咳嗽患者病久肺气虚损、痰瘀毒蕴的基本病理要素,认为该病气虚运化无力则生痰致瘀,痰瘀搏结日久而内蕴毒邪,提出应以益气解毒法为纲领治疗,擅用黄芪、金银花、当归、甘草、浙贝母、瓜蒌皮、穿山龙、石韦等益气解毒之要药来干预IPF咳嗽。选取周平安教授在既往临床治疗IPF咳嗽中的一经典临证医案,详细分析案例中患者病情转归过程中的各阶段用药思路,对周平安教授以益气解毒法应对IPF咳嗽的辨证治疗经验进行分析总结,为中医药防治特发性肺纤维化咳嗽提供借鉴和参考。This paper introduces the clinical experience of Professor Zhou Ping'an,an expert famous for treating respiratory warm diseases,in treating cough caused by lung impediment with the method replenishing qi and removing toxin.Professor Zhou believes that the cough caused by idiopathic pulmonary fibrosis(IPF)should be taken as the cough caused by lung impediment in traditional Chinese medicine(TCM)according to its syndrome characteristics.In the syndrome identification and treatment of this disease,attention should be paid to the basic pathological elements of lung qi deficiency and toxin caused by phlegm and stasis in patients.The qi deficiency caused by the disease will produce phlegm and stasis,the long-term accumulation of which generate toxin.Accordingly,he proposed the treatment method of replenishing qi and removing toxin.Specifically,Astragali Radix,Lonicerae Japonicae Flos,Angelicae Sinensis Radix,Glycyrrhizae Radix et Rhizoma,Fritillariae Thunbergii Bulbus,Trichosanthis Pericarpium,Dioscoreae Nipponicae Rhizoma,and Pyrrosiae Folium can be used to intervene in IPF-associated cough.Furthermore,this paper detailed the medication ideas at each disease stage of a classic case of IPF-associated cough treated by Professor Zhou,and summarized Professor Zhou's experience in the syndrome differentiation and treatment of this disease by replenishing qi and removing toxin method.This paper aims to provide a reference for the TCM prevention and treatment of IPF-associated cough.
