摘　　要：目的:分析残疾状况对我国老年人抑郁症状的影响及其内在机制,为了解和预防老年人抑郁症状提供参考。方法:利用2018年中国健康与养老追踪调查数据,运用二元logistic回归模型分析残疾对抑郁症状的作用,通过倾向值匹配法进行稳健性检验,采用因果模型逐步检验回归系数法和Bootstrap方法进行中介效应检验。结果:2018年,在7736名研究对象中,患有抑郁症状的老年人共有1774人(22.93%)。残疾状况对老年人抑郁症状具有影响,患有躯体(OR=1.75)、智力(OR=1.75)、视力(OR=1.60)和听力残疾(OR=1.38)的老年人具有高抑郁症状风险(P<0.01),日常生活活动能力对其有中介效应(P<0.01)。结论:残疾状况影响老年人抑郁症状,日常生活活动能力在其中起中介作用。残疾预防和日常生活活动能力的改善有助于降低抑郁风险。Objective:To analyze the impact of disability on depressive symptoms in Chinese older adults and its underlying mechanisms to provide evidence for understanding and preventing depression.Methods:Using data from the 2018 China health and retirement longitudinal study,the binary logistic regression model was used to explore the impact of disability on depressive symptoms.The robustness test was conducted by the propensity score matching(PSM)method.This study applied the stepwise test of regression coefficient in causal model and the boostrap method to analyze the meditating effect.Results:In 2018,among 7736 people,there were 1774(22.93%)elderly people with depressive symptoms.Disability had a significant effect on depressive symptoms in older adults,with those with physical(OR=1.75),intellectual(OR=1.75),visual(OR=1.60),and hearing(OR=1.38)disabilities having a high risk of depressive symptoms(P<0.01).In addition,activities of daily living mediated this relationship(P<0.01).Conclusions:Disability affects depressive symptoms in older adults,with activities of daily living as the mediator.The prevention of disability and the improvement of ability to perform activities of daily living can help to reduce the risk of depressive symptoms.
