基于古代文献的中医治疗口腔溃疡用药规律研究

摘　　要：目的基于古代口腔溃疡治疗,探讨口腔溃疡治疗用药规律。方法收集《中华大典·医药卫生典·医学分典·耳鼻咽喉口齿总部》中“口疮”“舌疮”治疗方药,采用Excel软件(版本16.5922031300)录入数据,统计单味药物用药频次、分类、药性、药味、归经;采用SPSS Statistics26.0软件对方药俱全的处方进行聚类分析,SPSS Modeler 18.0软件进行关联规则分析。结果共涉及中药228味,总频次1974次,频次>5的中药74味,总频次1122次。使用频次较高的单味药依次为黄柏、黄连、甘草、附子、麦冬等,功效类别以清热药、补虚药为主,性味以寒、苦为多,归经主要为脾、肺、心、胃经;对方药俱全方剂中的药物采用聚类分析及关联规则分析,分别得到4个聚类及17个潜在药物组合;发现了现代较少使用但有应用潜力的蔷薇根、角蒿、大青盐、硝石等溃疡治疗药物。结论古代中医治疗口腔溃疡以清热为主,补虚(尤其是补阳)治法亦较多,提示口腔溃疡证候多样,临床应辨证治疗;蔷薇根、角蒿、大青盐、硝石等药物在口腔溃疡治疗中的临床和研究意义值得关注。Objective To discuss the medication law of the treatment for oral ulcer based on the ancient oral ulcer treatment.Methods The drugs and prescriptions of“oral ulcer”and“tongue ulcer”in A Branch of Great Dictionary of Chinese Culture,Dictionary of Medicine and Health Care,Branch Dictionary of Medicine,Headquarters of Ear,Nose,Throat,Mouth and Teeth were collected and recorded in Excel.The frequency,classification,property,taste and meridians of each drug were counted.SPSS Statistics 26.0 was used for clustering analysis of complete prescriptions,and SPSS Modeler 18.0 was used for association rule analysis.Results Totally 228 kinds of Chinese materia medica for the the treatment of oral ulcer were obtained,with a total frequency of 1974 times,and 74 kinds of Chinese materia medica with frequency were greater than or equal to 5,with a total frequency of 1122 times.The highest-frequency medicines were Phellodendri Chinensis Cortex,Coptidis Rhizoma,Glycyrrhizae Radix et Rhizoma,Aconiti Lateralis Radix Praeparata,and Ophiopogonis Radix.The drugs for clearing heat,and tonifying deficiency were the most frequently application.The properties and tastes were mainly cold and bitter.The meridians were mainly splen,lung,heart and stomach meridians.Using clustering analysis to analyze the drugs in complete prescriptions,four drug combinations were obtained;using association rule analysis to analyze the drugs in complete prescriptions,17 potential drug combinations were obtained;modern ulcer treatment drugs that are less commonly used but have potential for application were discovered,such as Rosa multiflora Thunb.,Incarvillea sinensis Lam.,Malitum,and Nitre.Conclusion Heat-clearing drugs are the most commonly used in the treatment of oral ulcer in ancient TCM,and drugs for tonifying deficiency,especially drugs for tonifying yang,are also widely used,suggesting that the syndrome of oral ulcer is diverse.Clinical treatment should be carried out based on syndrome differentiation.The clinical and research significance of dr
