主动脉弓左侧静脉变异的MSCT诊断价值

摘　　要：目的探讨主动脉弓左侧静脉变异的CT影像特征,提高诊断及鉴别诊断的能力,避免误诊漏诊。方法选取我院住院患者经彩色多普勒超声心动图及CT共同诊断的18例主动脉弓左侧静脉变异的CT图像。结果心上型完全型肺静脉异位引流2例,左上肺部分型肺静脉异位引流2例,永存左上腔静脉11例,主动脉弓下左头臂静脉2例,食管后左头臂静脉1例。结论主动脉弓左侧异常血管断面是心上型完全型肺静脉异位引流、左上肺部分型肺静脉异位引流、永存左上腔静脉、主动脉弓下左头臂静脉、食管后左头臂静脉的共同CT征象,不同的是血管走行,通过此征象可以提高CT对主动脉弓旁先天静脉畸形的诊断水平,避免误诊漏诊。Objective To investigate the CT imaging features of left aortic arch vein variation,to improve the ability of diagnosis and differential diagnosis,and to avoid misdiagnosis and missed diagnosis.Methods The CT images of 18 cases of left aortic arch venous variation confirmed by echocardiography and CT in our hospital were analyzed retrospectively.Results Supracardiac total anomalous pulmonary venous drainage was showen in 2 cases,partial anomalous pulmonary venous drainage from the upper lobeof theleft lung in 2 cases,persistent left superior vena cava in 11 cases.There were 2 cases of the left brachiocephalic vein beneath the aorticarch,and 1 case of the retroesophageal left brachiocephalic vein.Conclusion The presence of vascular cross section on the left side of aortic arch is the common CT signs of supracardiac total anomalous pulmonary venous drainage,partial anomalous pulmonary venous drainage of the left upper lung,persistent left superior vena cava,and the left innominate vein low and retroesophageal innominate vein,but,the difference is vein route.This sign can improve the diagnostic level of CT for congenital venous malformation and avoid misdiagnosis and missed diagnosis.
