摘 要:目的探讨彩色多普勒超声检查评价左肾静脉回流受阻后发生自发性分流途径中的价值。方法选取应用彩色多普勒超声检查并经临床证实为胡桃夹综合征或者左肾静脉血栓所致肾静脉回流障碍患者62例(观察组)进行分析,观察其自发性分流发生与否、引流途径及血流方向,并与同期不伴有肾静脉回流障碍志愿者50例(对照组)的肾静脉超声影像对照。结果观察组中超声检查显示侧支循环建立17例,对照组中检测到侧支循环建立2例,两组之间对比差异有统计学意义(P=0.001)。结论彩色多普勒超声检查可以显示发生回流障碍时左肾静脉血流受阻的直接征象、间接征象及血流动力学改变,为肾静脉回流受阻后评估左肾功能提供影像学依据。Objective Color Doppler ultrasound was used to evaluate the spontaneous return pathway established after obstruction of left renal vein reflux.Methods Color Doppler ultrasound imaging data were analyzed on 62 patients,who were confirmed clinically with left renal vein reflux disorder caused by nutcracker syndrome or left renal vein thrombosis.The occurrence of spontaneous shunt,drainage pathway and blood flow direction were observed and compared with the renal vein ultrasound images of 50 volunteers without renal vein reflux disorder.Results The establishment of collateral circulation was detected by ultrasound in 17 cases in the experimental group,whereas,2 cases in the control group.The difference between these two group was statistically significant(P=0.001).Conclusion Color Doppler ultrasound can present the direct signs,indirect signs and hemodynamic changes of left renal vein blood flow when renal vein reflux obstruction occurs,so as to provide imaging basis for evaluating left renal function after renal vein reflux obstruction.
