摘 要:目的 比较细胞培养和分子生物学方法在急性弛缓性麻痹(Acute flaccid paralysis, AFP)病例病原学监测中的应用。方法 利用细胞培养和分子生物学方法对广东省2015-2017年AFP病例粪便标本进行脊髓灰质炎病毒(Poliovirus, PV)和非脊髓灰质炎肠道病毒(Non-polio enterovirus, NPEV)检测,比较两种检测方法的阳性率。结果 在2015-2017年1 156份AFP病例粪便标本中,细胞培养、分子生物学方法检测PV阳性率分别为1.90%、2.42%,NPEV阳性率分别为5.97%、14.71%。分子生物学方法相对于细胞培养方法检测PV的灵敏度和特异度分别为100%和99.50%,检测NPEV分别为100%和90.70%。结论 分子生物学方法较细胞培养方法的PV和NPEV检出率高。在相关聚集性疫情处置中可联合使用两种方法以提高检测灵敏度和特异度。Objective To compare cell culture and a molecular biological technique in pathogenic surveillance of acute flaccid paralysis(AFP)cases.Methods We used both methods to detect polioviruses(PV)and non-polio enteroviruses(NPEV)in fecal samples from AFP cases collected during 2015 to 2017 in Guangdong province.We compared PV and NPEV positivity between the two methods.Results In 1156 fecal samples,PV positivity was 1.90% when using cell culture and was 2.42% when using the molecular biological technique;respective NPEV positivity rates were 5.97% and 14.71%.Using cell culture as the gold standard,the sensitivity of the molecular biological technique was 100% and the specificity was 99.50% for PV detection;for NPEV detection,sensitivity was 100% and specificity was 90.70%.Conclusions PV and NPEV positivity rates using the molecular biological technique were higher than when using cell culture.Combination of the two methods can increase sensitivity and specificity of detection in responses to clustering cases.
