伤科黄水冷敷联合小夹板超跟背伸固定治疗急性踝关节外侧副韧带损伤的价值探究

摘　　要：目的 分析伤科黄水冷敷联合小夹板超跟背伸固定治疗急性踝关节外侧副韧带损伤的价值。方法 选取我院收治的90例急性踝关节外侧副韧带损伤Malliaropoulos Ⅱ级患者,运用随机数余数法分为对照组、小夹板组与联合组三组。对照组(30例)采用常规西医治疗,小夹板组(30例)在对照组基础上联合小夹板超跟背伸固定,联合组(30例)在对照组基础上用伤科黄水冷敷联合小夹板超跟背伸固定治疗。比较三组患者治疗效果,治疗前后踝关节症状积分、踝关节功能和患肢运动能力,患肢肿胀消退率以及治疗前后患肢疼痛评分。结果 治疗后,三组临床症状均有所改善,其中联合组踝关节症状积分低于小夹板组与对照组(P <0.05),踝关节功能评价量表评分高于小夹板组与对照组(P <0.05),患肢腓总神经与胫总神经传导速度高于小夹板组与对照组(P <0.05),视觉模拟量表评分法评分低于小夹板组与对照组(P <0.05),联合组临床总有效率为96.67%,高于小夹板组的80.00%与对照组的66.67%(P <0.05),而小夹板组与对照组上述指标差异则皆无统计学意义(P> 0.05)。结论 伤科黄水冷敷联合小夹板超跟背伸固定治疗急性踝关节外侧副韧带损伤疗效显著,可明显改善患者患肢临床症状与踝关节功能,值得推广。Objective To analyze the value of Shangke Huangshui cold compress combined with small splint super-heel extension fixation in treating acute injury of lateral collateral ligament of the ankle joint.Methods Ninety patients with Malliaropoulos II grade acute injury of lateral collateral ligament of the ankle joint treated in Sanshui Hospital were randomized into control(routine western medicine),small splint fixation(small splint super-heel extension fixation),and combined treatment(Shangke Huangshui cold compress combined with small splint super-heel extension fixation)groups(n=30).The therapeutic effects of the three groups were compared.The TCM symptom score,ankle function,motor ability of the affected limb,swelling disappearance rate of the affected limb,and visual analogue scale(VAS)score of the affected limb were determined before and after treatment.Results After treatment,the clinical symptoms of the three groups were alleviated.The combined treatment group had lower TCM symptom score,higher ankle joint function score,higher conduction velocity of common peroneal nerve and common tibial nerve in the affected limb,and lower VAS score than the other two groups(all P<0.05).The total response rate of the combined treatment group was 96.67%,which was higher than those of the small splint fixation group(80.00%)and the control group(66.67%)(P<0.05).Th above indexes showed no significant differences between the small splint fixation group and the control group(P>0.05).Conclusion Shangke Huangshui cold compress combined with small splint super-heel extension fixation is effective in the treatment of acute injury of lateral collateral ligament of the ankle joint.This therapy can alleviate the clinical symptoms and improve the ankle function of the patients,which is worth popularizing.
