摘 要:目的探讨精子核成熟度与精子数量、活力、形态学和DNA完整性的相关性。方法回顾性分析2022年1月1日至12月31日在深圳中山泌尿外科医院行精子核成熟度检查的423例男性患者的临床资料,根据精子核成熟度不同将患者分为高成熟组(136例)、中成熟组(242例)和低成熟组(45例)。比较各组患者的精液常规参数(精子总数、精子浓度、前向运动精子百分率等)、DNA碎片指数(DFI)、顶体酶活性等,采用双变量、偏相关和线性回归分析精子核成熟度与精液质量的相关性。结果各组间精液常规参数比较:低成熟组的精子总数、精子浓度显著低于高成熟组(P<0.05);低成熟组和中成熟组的前向运动精子百分率显著低于高成熟组(P<0.05),非活动精子百分率显著高于高成熟组(P<0.05);正常形态精子百分率随着核成熟度的降低而显著降低(P<0.05),头部缺陷精子百分率随着核成熟度的降低而显著升高(P<0.05)。低成熟组和中成熟组的DFI和高DNA染色(HDS)显著高于高成熟组(P<0.05),低成熟组的HDS显著高于中成熟组(P<0.001);各组间精子顶体酶活性和活性氧(ROS)水平比较无显著性差异(P>0.05)。相关性分析结果显示:校正男方年龄因素后,除精子浓度与精子核成熟度未表现出明显相关性(P>0.05)外,精子总数、前向运动精子百分率、正常形态精子百分率与精子核成熟度均呈正相关(P<0.05),不活动精子百分率、头部缺陷精子百分率、DFI及HDS与精子核成熟度均呈负相关(P<0.05)。结论精子核成熟度与精子活力、形态学、DFI及HDS均存在一定相关性,尤其与HDS显著相关,提示精子核成熟度可以作为精液检查常规参数的补充,值得临床关注。Objective:To investigate the correlation between sperm nuclear maturity and semen quality including sperm count,sperm motility,sperm morphology and sperm DNA integrity.Methods:The clinical data of 423 male patients who were detected sperm nuclear maturity in Shenzhen Zhongshan Urology Hospital from 1 st January 2022 to 31 st December 2022 were retrospectively analyzed.These patients were divided into high maturity group(n=136),medium maturity group(n=242)and low maturity group(n=45)according the level of sperm nuclear maturity.The semen routine parameters including total sperm count,sperm concentration and percentage of progressive sperm motility,and DNA fragmentation index(DFI)and acrosin activity etc.were analyzed.Then bivariate,partial correlation and linear regression analysis were used to assess the correlation between sperm nuclear maturity and semen quality.Results:Comparing the difference of semen parameters among the groups showed that total sperm count and sperm concentration in low maturity group were significantly lower than high maturity group(P<0.05).The percentage of progressive sperm motility in low and medium maturity group were significantly lower than high maturity group(P<0.05),while percentage of immotile sperm was significantly higher than high maturity group(P<0.05).The percentage of morphologically normal spermatozoa significantly decreased with reduction of nuclear maturity(P<0.05),while the percentage of sperm with head defects significantly increased with decrease of nuclear maturity(P<0.05).The level on DFI and high DNA stainability(HDS)in low and medium maturity group were significantly higher than high maturity group(P<0.05),and HDS in low maturity group were significantly higher than medium maturity group(P<0.001).No significant difference was observed in acrosin activity and reactive oxygen species among the groups(P>0.05).The correlation analysis results show that there was no significantly correlation between sperm nuclear maturity and sperm concentration after adjusting male age
