摘  要:目的研究荷叶黑茶提取物对内生湿热证患者肠道菌群、临床症状及糖脂代谢等相关指标的影响。方法采用自身前后对照的方法,选取2022年4—7月江苏省第二中医院门诊内生湿热证患者23例,服用荷叶黑茶提取物(6 g/d),治疗1个月后评估其临床疗效和不良反应,并通过Illumina MiSeq平台对患者治疗前后的肠道菌群进行16S rDNA基因V3~V4可变区检测,运用QIIME、STAMP及LEfSe进行Alpha、Beta多样性分析,鉴定差异性菌属。结果内生湿热症候经治疗显著改善(有效率>60%);体质量指数(BMI)、体脂率、腰臀脂肪比、空腹血糖(FPG)、空腹胰岛素(FINS)、胰岛素抵抗(IR)、总胆固醇(TC)、甘油三酯(TG)和低密度脂蛋白(LDL-C)均较治疗前下降,差异有统计学意义(P<0.05);2例(8.70%)患者在服药期间有失眠症状;与治疗前相比,治疗后蓝藻菌门(Cyanobacteria)的相对丰度上升,拟杆菌门(Bacteroidetes)的相对丰度下降,差异有统计学意义(P<0.05);属分类水平上,罗斯氏菌属(Roseburia)、克雷伯氏菌属(Klebsiella)的相对丰度显著上升,吉米菌属(Gemmiger)、WAL-1855D、卟啉单胞菌属(Porphyromonas)的相对丰度显著下降,差异有统计学意义(P<0.05)。结论荷叶黑茶提取物可以明显改善内生湿热证的临床症状,调整糖脂代谢指标,提高胰岛素敏感性,其可能通过恢复肠道菌群的稳态发挥调节内生湿热证作用。Objective To investigate the effects of extracts from lotus leaf and black tea on intestinal flora,clinical symptoms,glucose and lipid metabolism in patients with endogenous damp-heat syndrome.Methods A total of patients with endogenous damp-heat syndrome selected in Jiangsu Second Chinese Medicine Hospital from April 2022 to July 2022 were treated with lotus leaf black tea extract(6 g/d).Evaluate its clinical efficacy and adverse reactions after 1 month of treatment.All patients of intestinal flora 16 S rDNA gene variable area(V3-V4)were detected through the Illumina MiSeq platform.The Alpha,Beta diversity analysis was detected through QIIME,differential flora between two groups were identified through STAMP and LEfSe.Results The patients'endogenous damp-heat syndrome improved significantly after treatment(effectiveness>60%).Body mass index(BMI),body fat rate and the waist hip fat,fasting plasma glucose(FPG),fasting insulin(FINS),insulin resistance(IR),total cholesterol(TC),triglyceride(TG)and low density lipoprotein(LDL-C)were decreased significantly(P<0.05).TWO patients(8.70%)had insomnia symptoms during taking the medicine,and the symptoms were relieved after adjusting the time of taking the medicine.Compared with before treatment,at the phylum level,the relative abundance of Cyanobacteria increased and that of Bacteroidetes decreased significantly after treatment(P<0.05).At the genus level,the relative abundance of Roseburia and Klebsiella increased significantly,while that of Gemmiger,WAL-1855D and Porphyromonas decreased significantly(P<0.05).Conclusion Extract of lotus leaf and black tea could significantly improve clinical symptoms,improve glucose and lipid metabolism indexes,and increase insulin sensitivity in patients with endogenous damp-heat syndrome.It may work by restoring the homeostasis of the intestinal flora.
