摘 要:目的:应用高频线阵探头测量胎儿胰腺各生物学参数,为产前筛查提供依据。方法:选取产前常规超声检查的540例正常胎儿,孕周16~37周,评价高频、低频探头对胰腺的显示率,并应用高频探头测量不同孕周胎儿胰头、胰体、胰尾的前后径,胰腺周长、面积及腹围。采用Pearson或Spearman相关分析法分析胎儿胰腺生物学参数与孕周及腹围的相关性。结果:540例正常胎儿中,胰腺清晰显示385例,其中高频探头显示率为71.11%(384/540),低频探头为69.07%(373/540),两者联合为71.30%(385/540)。高频探头对妊娠16~26周胎儿胰腺的显示率与低频探头相比,具有一定优势(87.44%vs. 80.26%)。胎儿胰头、胰体、胰尾的前后径及胰腺周长、面积均与孕周及腹围呈明显正相关(r值0.864~0.962)。结论:胎儿胰腺大小与孕周、腹围均呈明显正相关。高频线阵探头所测胰腺数值可为评估胎儿胰腺发育、早期发现胰腺发育异常提供诊断依据。Objective:To investigate the growth and development of the fetal pancreas by measuring the size of the fetal pancreas with a high-frequency linear array probe.Methods:A total of 540 normal fetuses who underwent prenatal ultrasound were selected.The gestational age of all fetuses ranged from 16 to 37 weeks.The display rate of the pancreas with high-frequency and low-frequency probes was evaluated.The anterior and posterior diameters,perimeter,area and abdominal circumference of the head,body and tail of the pancreas of fetuses at different gestational weeks were measured with high-frequency probes.Pearson’s method or Spearman correlation analysis were used to analyze the correlation between the biological parameters of the fetal pancreas and gestational age and abdominal circumference.Results:Of the 540 fetuses,385 fetuses could clearly display the pancreas,the high-frequency display rate was 71.11%(384/540),the low-frequency display rate was 69.07%(373/540),and the combined display rate of the two probes was approximately 71.30%(385/540).The high-frequency probe had a significant advantage in displaying the pancreas at 16~26 weeks gestation(87.44%vs.80.26%).The anteroposterior diameter,perimeter and area of the head,body and tail of pancreas were positively correlated with gestational age and abdominal circumference(r values ranged from 0.864 to 0.962).Conclusions:The fetal pancreas size is positively correlated with gestational age and abdominal circumference,and the fetal pancreas parameters measured with a high-frequency linear array probe can provide a diagnostic basis for evaluating fetal pancreas development and early detection of congenital pancreatic abnormalities.
