无会阴侧切低位产钳术在产妇分娩中的应用

摘　　要：目的探讨无会阴侧切低位产钳术在产妇分娩中的应用效果。方法选取100例行无会阴侧切低位产钳术产妇为研究组,另选取同期100例行会阴侧切低位产钳术产妇为对照组。比较两组产后2h、24h出血量、伤口裂伤程度、伤口愈合情况、伤口疼痛感、产后住院时间及费用、新生儿产伤、新生儿窒息的差异。结果研究组产后2h出血量少于对照组(P<0.05),产后24h出血量比较,差异无统计学意义(P>0.05);研究组伤口疼痛评分低于对照组(P<0.05);研究组住院费用少于对照组(P<0.05);两组产后住院时间比较,差异无统计学意义(P>0.05);研究组伤口裂伤程度优于对照组(P<0.05);两组新生儿窒息率比较,差异无统计学意义(P>0.05);两组均为甲级愈合,均无新生儿产伤情况发生。结论无会阴侧切低位产钳术在产妇分娩中的应用效果较好,不会增加母婴并发症发生,能降低产后出血量、会阴伤口愈合不良及新生儿窒息率,同时还可以缩短住院时间,减轻患者疼痛、经济及心理负担,促进母婴康复,值得临床推广与应用。Objective To investigate the effect of low forceps delivery without lateral perineal incision in parturient women.Methods A total of 1oo0 parturients without lateral perineal resection with low forceps were selected as the study group and 100 parturients with lateral perineal resection with low forceps were selected as the control group.The differences of 2 h and 24 h postpartum blood loss,wound laceration degree,wound healing,wound pain,postpartum hospital stay and cost,neonatal birth injury and neonatal asphyxia were compared between the two groups.Results Two hours postpartum blood loss in the study group was less than that in the control group(P<0.05),and 24 h postpartum blood loss was not statistically significant(P>0.05).The wound pain score of the study group was lower than that of the control group(P<0.05).The study group cost less than the control group(P<0.05).There was no significant difference in postpartum hospital stay between the two groups(P>0.05).The wound laceration degree of the study group was better than that of the control group(P<0.05).There was no significant difference in neonatal asphyxia rate between the two groups(P>0.05).Both groups had Grade A healing and no neonatal injury occurred.Conclusion The application of low forceps without lateral perineal incision in maternal delivery has a good effect,does not increase the occurrence of maternal and infant complications,can reduce postpartum blood loss,poor perineal wound healing and neonatal asphyxia rate,and can shorten the length of hospital stay,reduce patients'pain,economic and psychological burden,and promote maternal and infant rehabilitation,which is worthy of clinical promotion and application.
