改良二瓣法手术联合GnRH-a、地诺孕素治疗子宫腺肌病的疗效观察

摘　　要：目的观察改良二瓣法手术联合促性腺激素释放激素激动剂(GnRH-a)、地诺孕素治疗子宫腺肌病的疗效。方法选取58例子宫腺肌病患者,按照随机数字表法分为对照组与试验组,各29例。对照组采用GnRHa+地诺孕素治疗,试验组采用改良二瓣法手术联合GnRH-a+地诺孕素治疗。对比两组临床疗效。结果治疗前,两组血清糖类抗原125(CA125)、血红蛋白(Hb)水平比较,差异无统计学意义(P>0.05);试验组治疗后3个月、6个月、12个月血清CA125水平低于对照组,Hb水平高于对照组(P<0.05)。治疗前,两组月经疼痛评分比较,差异无统计学意义(P>0.05);试验组治疗后月经疼痛评分(1.33±0.43)分低于对照组的(2.22±0.76)分(P<0.05)。治疗前,两组月经量评分与子宫体积比较,差异无统计学意义(P>0.05);试验组治疗后月经量评分低于对照组,子宫体积小于对照组(P<0.05)。结论在子宫腺肌病患者治疗过程中,采用改良二瓣法手术联合GnRH-a、地诺孕素治疗方案,可提升治疗效果,从而达到缓解疼痛与促进术后恢复的目的,值得临床推广。Objective To observe the efficacy of modified two-valve surgery combined with gonadotropin-releasing hormone agonist(GnRH-a)and dinorgestrel in the treatment of adenomyopathy.Methods 58 patients with adenomyopathy were selected and divided into control group and experimental group according to random number table method,with 29 cases in each group.The control group was treated with GnRH-a+dinorgestrel,and the experimental group was treated with modified two-valve surgery combined with GnRH-a+dinorgestrel.The clinical eficacy of the two groups was compared.Results Before treatment,there was no significant difference in serum carbohydrate antigen 125(CA125)and hemoglobin(Hb)levels between the two groups(P>0.05).At 3 months,6 months and 12 months after treatment,the serum CA125 level of experimental group was lower than that of control group,and Hb level was higher than that of control group(P<0.o5).Before treatment,there was no significant difference in menstrual pain scores between the two groups(P>0.05).The dysmenorrhea score of the experimental group(1.33±0.43)points was lower than that of the control group(2.22±0.76)points(P<0.05).Before treatment,there was no significant difference in menstrual volume score and uterine volume between the two groups(P>0.05).After treatment,the score of menstrual volume in experimental group was lower than that in control group,and the uterine volume was lower than that in control group(P<0.05).Conclusion In the treatment of patients with adenomyopathy,the modified two-valve surgery combined with GnRH-a and dinorgestrel can improve the therapeutic effect,thus achieving the purpose of relieving pain and promoting postoperative recovery,and is worthy of clinical promotion.
